摘　　要：目的分析河北省产妇分娩年龄与妊娠结局的关系,为高危妊娠的围产保健工作提供依据。方法收集河北省22个危重孕产妇监测点2016年的61 017例单胎产妇资料,采用Logistic回归模型研究分娩年龄与不良妊娠结局发生风险的关系。结果 2016年河北省产妇的平均分娩年龄为(28.4±4.4)岁,分娩年龄<20岁者630例(1.0%),≥35岁高龄妊娠者5 905例(9.7%),其中分娩年龄≥40岁者796例(1.3%)。与25~30岁比较,分娩年龄<20岁组,20~25岁组发生妊娠期糖尿病、剖宫产的风险显著降低,但子痫前期重度、早产、小于孕龄儿的发生风险显著增加;分娩年龄30~35岁组、35~40岁组,≥40岁组妊娠期糖尿病、子痫前期轻度、子痫前期重度、早产、剖宫产的发生风险均显著增高(P<0.05)。结论高龄妊娠和低龄妊娠均可明显增加妊娠不良结局的发生风险,应加强其围产保健工作。Objective To investigate the correlation between maternal age and pregnancy outcomes in Hebei province in order to provide a reference for the health service of high-risk pregnancy women at perinatal stage.Methods The clinical data about 1017 singleton pregnancy parturients from 22 critical maternal monitoring sites in Hebei province were collected,and the correlation between maternal age and pregnancy outcomes was nalyzed by means of 1 ogistic regression model.Results The average maternal age at delivery in 2016 in Hebei province was(28.4±4.4) years of age,of whom,630 women(1.0%) delivered under 20 years of age,and 5,905 women(9.7%) delivered at 35 years and older.As compared with those in 25~30 years group, the incidence rates of GDM and cesarean section rate in <20 years group and 20~25 years group were significantly decreased,however,the incidence rates of severe preeclampsia, premature birth, fetus small-for gestational age were significantly increased.Moreover the incidence rates of GDM,preeclampsia and severe preeclampsia,preterm birth and cesarean section in 30~35 years group and 35~40 years group as well as and ≥40 years groups were significantly increased(P<0.05).Conclusion Both advanced age pregnancy and low age pregnancy can increase the risk of adverse pregnancy outcomes,thus, it is necessary to strengthen health service at perinatal stage.
