PFNA内固定治疗股骨粗隆间骨折的临床疗效探讨

摘　　要：目的探讨PFNA内固定治疗股骨粗隆间骨折的临床疗效。方法随机选取2013年8月—2016年8月福建中医药大学附属福鼎市医院骨科股骨粗隆间骨折患者60例,随机分为两组:一组防旋型股骨近端髓内钉内固定术治疗组(PFNA内固定组,30例),一组动力髋螺钉内固定术治疗组(DHS内固定组,30例),统计分析两组患者的手术相关指标、临床疗效、术后并发症发生情况。结果PFNA内固定组患者的手术时间、术后下床活动时间、骨折愈合时间均显著短于DHS内固定组(t=6.965、3.365、3.306,P<0.05),术中出血量显著少于DHS内固定组(t=4.541,P<0.05),Harris评分显著高于DHS内固定组(t=4.303,P<0.05),髋关节功能恢复优良率96.7%(29/30)显著高于DHS内固定组86.7%(26/30)(χ^2=12.830,P<0.05),术后并发症发生率3.3%(1/30)显著低于DHS内固定组16.7%(5/30)(χ^2=13.340,P<0.05)。结论PFNA内固定治疗股骨粗隆间骨折的临床疗效较DHS内固定显著。Objective To investigate the clinical efficacy of PFNA internal fixation for intertrochanteric fractures.Methods Sixty patients with skeletal intertrochanteric fractures from Fuding Hospital of Fujian University of Traditional Chinese Medicine from August 2013 to August 2016 were randomly selected and randomly divided into two groups:a group of anti-spin type proximal femoral nails.The fixed treatment group(30 patients in the PFNA internal fixation group),30 patients,a group of dynamic hip screw internal fixation(DHS internal fixation group,30 cases),statistical analysis of the two groups of patients with surgical related indicators,clinical efficacy,postoperative complications occurrence of the disease.Results The operation time,postoperative outpatient activity time,and fracture healing time of patients in the PFNA internal fixation group were significantly shorter than those in the DHS internal fixation group(t=6.965,3.365,3.306,P<0.05).The intraoperative blood loss was significantly less than that DHS internal fixation group(t=4.541,P<0.05),Harris score was significantly higher than DHS internal fixation group(t=4.303,P<0.05),and hip joint function recovery rate was 96.7%(29/30)significantly higher than DHS The internal fixation group was 86.7%(26/30)(χ^2=12.830,P<0.05),and the postoperative complication rate was 3.3%(1/30)significantly lower than that in the DHS internal fixation group[16.7%(5/30)](χ^2=13.340,P<0.05).Conclusion The clinical efficacy of PFNA internal fixation for the treatment of intertrochanteric fractures is significantly higher than that of DHS.
