PFNA治疗老年股骨粗隆间骨折疗效分析

摘　　要：目的分析老年股骨粗隆间骨折患者采取PFNA治疗的临床效果。方法该次方便选择2015年1月—2018年12月该院收治股骨粗隆间骨折患者61例为研究对象,参照计算机抽选结果分为观察组与对照组,对照组患者采取钢板固定术治疗,观察组患者采取PFNA治疗,对比临床疗效,统计并分析两组患者手术相关指标差异,记录并计算各组并发症发生率。结果观察组临床总有效率明显优于对照组(χ^2=6.66,P<0.05),观察组患者手术时长(48.52±1.32)min、术后卧床时长(4.04±0.21)周、负重时间(1.75±0.14)周、骨折愈合时间(13.25±1.42)周均明显短于对照组,且术中出血量(252.49±10.22)mL、术后引流量(55.94±2.14)mL均少于对照组,切口长度(5.32±1.01)cm短于对照组(t=25.48、20.150、8.740、9.280、24.330、30.040、9.330,P<0.05)。观察组患者总并发症发生率较对照组相比更低(P<0.05)。结论老年股骨粗隆间骨折患者采取PFNA治疗效果更佳,具有切口短、出血量少、恢复快、并发症少等优点,值得临床推广应用。Objective To analyze the clinical effect of PFNA in elderly patients with intertrochanteric fractures.Methods 61 patients with femoral intertrochanteric fractures in the hospital from January 2015 to December 2018 were convenient selected as subjects.The results of computer sampling were divided into observation group and control group.The control group was treated with plate fixation.Patients in the observation group were treated with PFNA.The clinical efficacy was compared.The differences in surgical related indexes between the two groups were statistically analyzed.The incidence of complications in each group was recorded and calculated.Results The total effective rate of the observation group was significantly better than that of the control group(χ^2=6.66,P<0.05).The length of operation(48.52±1.32)min,the length of bed rest after surgery(4.04±0.21)weeks,and the weight-bearing time(1.75±0.14)weeks,fracture healing time(13.25±1.42)weeks were significantly shorter than the control group,and intraoperative blood loss(252.49±10.22)mL,postoperative drainage volume(55.94±2.14)mL were less than the control group,the length of the incision(5.32±1.01)cm shorter than the control group(t=25.480,20.150,8.740,9.280,24.330,30.040,9.330,P<0.05).The incidence of total complications was lower in the observation group than in the control group(P<0.05).Conclusion The treatment of senile femoral intertrochanteric fracture is better with PFNA.It has the advantages of short incision,less bleeding,quick recovery and less complications.It is worthy of clinical application.
