防盲超乳白内障术中后囊膜破裂一期植入后房型人工晶体的临床观察

摘　　要：目的观察防盲超乳术中后囊膜破裂一期植入后房型人工晶体的临床疗效。方法对33例33眼后囊膜破裂的患者行人工晶体一期植入。其中18例直接将人工晶体植入睫状沟内,9例采用单襻睫状沟缝线固定法,3例采用四襻睫状沟缝线固定法,3例采用巩膜层间无缝线固定法。术后随访6-12月,观察视力、眼压、人工晶体稳定性及并发症。结果33眼术后视力均较术前有不同程度的提高(矫正视力≥0.6者18例,0.2~0.6者13例,<0.2者2例),人工晶体无明显偏位及倾斜,眼压正常,未见有视网膜脱离、角膜失代偿等并发症。结论超声乳化白内障术中发生后囊膜破裂,采用合适的处理方法,一期植入人工晶状体,可以达到较好的临床疗效。Objective To observe the clinical effect of primary implantation of posterior chamber intraocular lens in cases of posterior capsule rupture during cataract phacoemulsification for prevention of blindness.Methods 33 eyes of 33 patients which sustained posterior capsule rupture happened during the surgery were followed up 6 month to12 months by observation of visual acuity,IOP,IOL stability and postoperative complications.Among all the cases,18 eyes were implanted directly into the ciliary sulcus and 9 eyes by one loop suture fixation and 3 eyes by four loop suture fixation and 3 eyes by intra sclera sutureless fixation.Results The postoperative best-corrected visual acuity was improved in 33 eyes,among which≥0.6 was in 13 eyes,0.2-0.6 in 6 eyes and<0.2 in 2 eyes.There was no obvious IOL decentration or tilting and no complications such as retinal detachment,corneal decompensation during the follow-up.The postoperative intraocular pressure was normal.Conclusions When posterior capsule rupture during cataract phacoemulsification,primary implantation of posterior chamber intraocular lens after suitable treatments can lead to a good clinical effect.
