摘 要:目的:探讨哮喘患儿外周血基质金属蛋白酶-9(MMP-9)、干扰素γ(IFN-γ)、巨噬细胞炎症蛋白-1α(MIP-1α)水平变化及其与肺功能的相关性。方法:选取86例支气管哮喘患儿为哮喘组,100例健康儿童为健康组。检测两组患者血清MMP-9、IFN-γ、MIP-1α水平和呼气峰流速(PEF)及第1秒用力呼气容积占肺活量的百分比(FEV1/FVC)等肺功能指标,探讨血清MMP-9、IFN-γ、MIP-1α与支气管哮喘患儿肺功能状况的相关性。结果:哮喘组患儿血清MMP-9、MIP-1α表达水平均高于健康组,而哮喘组血清IFN-γ水平及肺功能PEF、FEV1/FVC则低于健康组(P<0.05)。哮喘患儿血清MMP-9表达水平与肺功能指标PEF、FEV1/FVC均呈显著负相关(r=-0.399,P<0.05;r=-0.407,P<0.05),MIP-1α水平与PEF、FEV1/FVC也呈显著负相关(r=-0.380,P<0.05;r=-0.346,P<0.05);而血清IFN-γ水平与PEF、FEV1/FVC呈显著正相关(r=0.460,P<0.05;r=0.431,P<0.05)。治疗后,治疗有效组患儿血清MMP-9、MIP-1α水平均明显低于治疗无效组,血清INF-γ水平则高于无效组(P<0.05)。结论:哮喘患儿血清MMP-9、IFN-γ、MIP-1α水平与肺功能状况密切相关,对其检测有助于对支气管哮喘患儿肺功能判断和治疗效果评估。Objective:To investigate the changes of matrix metalloproteinase-9(MMP-9),interferon-γ(INF-γ),macrophage inflammatory protein-1α(MIP-1α)in peripheral blood of asthmatic children and the correlation with lung function.Methods:86 children with bronchial asthma were selected as asthma group and 100 healthy children as healthy group.Serum MMP-9,INF-γ,MIP-1αlevels,PEF and FEV1/FVC were measured.The correlation of serum MMP-9,INF-γ,MIP-1αin bronchial asthma with pulmonary function in children were investigated.Results:The expression levels of MMP-9,MIP-1αin asthmatic group were higher than those in healthy group,while the serum INF-γand PEF,FEV1/FVC in asthma group were lower than those in healthy group(P<0.05).The expression level of serum MMP-9 in asthmatic children was significantly negatively correlated with PEF and FEV1/FVC(r=-0.399,P<0.05;r=-0.407,P<0.05),the level of MIP-1αwas significantly negatively correlated with PEF and FEV1/FVC(r=-0.380,P<0.05;r=-0.346,P<0.05),while the level of serum INF-γwas positive correlated with PEF and FEV1/FVC(r=0.460,P<0.05;r=0.431,P<0.05).After treatment,the serum levels of MMP-9,MIP-1αin effective group were significantly lower than those in ineffective group,while the level of INF-γwas significantly higher than ineffective group(P<0.05).Conclusion:Serum MMP-9,INF-γ,MIP-1αlevels are closely related to pulmonary function in asthmatic children.It is helpful to diagnose and evaluate the treatment effect of bronchial asthma.
