股前外侧皮瓣游离移植术修复小腿及踝部皮肤软组织缺损临床疗效研究

摘　　要：目的:探讨股前外侧皮瓣游离移植术修复小腿及踝部皮肤软组织缺损的临床疗效。方法:选取86例小腿及踝部皮肤软组织缺损患者,随机分为观察组和对照组,每组各43例。对照组以常规背阔肌皮瓣移植修复治疗,观察组以股前外侧皮瓣游离移植修复治疗,对比两组修复效果、疼痛、手术指标和并发症发生情况。结果:观察组手术时间短于对照组(P<0.05),而两组术中出血量和住院时间对比无统计学差异(P>0.05);观察组皮瓣存活率显著高于对照组(P<0.05);两组治疗后皮瓣感觉和供区变化对比无统计学差异(P>0.05);两组不良反应发生情况对比无统计学差异(P>0.05)。结论:在小腿及踝部皮肤软组织缺损中以股前外侧皮瓣游离移植修复治疗可以缩短手术时间,提高皮瓣存活率,效果显著。Objective:to analyze the clinical effect of free transplantation of anterolateral femoral flap in repairing the skin and soft tissue defects of the lower leg and ankle.Methods:86 patients admitted with skin and soft tissue defects of leg and ankle were randomly divided into observation group and control group with 43 cases each.The control group was treated with conventional latissimus dorsi flap transplantation and the observation group was treated with free femoral anterolateral flap transplantation.Results:the operation time of the observation group was shorter than that of the control group(P<0.05),while there was no significant difference in intraoperative blood loss and hospital stay between the two groups(P>0.05).The survival rate of flap in observation group was significantly higher than that in control group(P<0.05).There was no significant difference in flap sensation and donor area change between the two groups after treatment(P>0.05).There was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion:the free transplantation of anterolateral femoral flap can shorten the operation time and improve the survival rate of skin and soft tissue defects in the lower leg and ankle.
