摘　　要：目的:探讨中老年原发性高血压(essential hypertension,EH)不同血压水平与血清催产素(oxytocin,OT)、胰岛素抵抗(insulin resistance,IR)的关系。方法:选取门诊体检健康者和EH患者作为研究对象,年龄≥45岁,分为以下四组:①体检健康组(正常对照组)50例;②血压1级组25例;③血压2级组22例;④血压3级组3例。观测四组年龄、腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、胰岛素(FINS)、胰岛素释放指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)、血清OT等指标。结果:正常对照组、EH1级组、EH2级组、EH3级组组间临血清OT、HOMA-IR与WHR、SBP、DBP、FPG、FINS、HOMA-β存在相关性(P≤0.001),血清OT与HOMA-IR呈负相关(r=-0.744),Logistic逐步回归分析显示,FPG、FINS、HOMA-IR、OT是EH的主要影响因素(P<0.05)。结论:中老年EH患者的血压水平与血清OT、IR具有相关性,建议在临床中加强该两项指标的监测。Objective To explore the relationship between different blood pressure levels and serum oxytocin insulin resistance in middle-aged and elderly patients with essential hypertension;Method The subjects were healthy persons and EH patients aged over 45 years.They were divided into four groups:①Physical examination group(normal control group),50 cases;②Blood pressure level 1 group,25 cases;③Blood pressure level 2 group,22 cases;④Blood pressure level 3 group,3 cases.Age,waist-hip ratio(WHR),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FPG),insulin(FINS),insulin release were observed.Radiation index(HOMA-beta),insulin resistance index(HOMA-IR),serum OT and other indicators;Results The correlation analysis showed that serum OT,HOMA-IR were correlated with WHR,SBP,DBP,FPG,FINS,HOMA-beta(P<0.001).Serum OT was negatively correlated with HOMA-IR(r=-0.744).Logistic stepwise regression analysis showed that FPG,FINS,HOMA-IR and OT were EH.Main influencing factors(P<0.05).Conclusion The blood pressure level of middle-aged and old EH patients is correlated with serum OT and IR.It is suggested to strengthen the monitoring of these two indicators in clinical practice.
