低温热塑板治疗儿童弹响拇的疗效观察

摘　　要：回顾性分析采用低温热塑板治疗107例(共115拇)儿童弹响拇的治疗效果。根据两种评定标准进行疗效评价。115拇指中,80拇(69.6%)治愈,20拇(17.4%)好转,15拇(13.0%)无效,总有效率(治愈+好转)为87.0%。根据Jung et al分级标准,治疗后0级的拇指数增多;而1级+2级的拇指数减少(P<0.05)。治疗后所有拇指的分级,相比治疗前没有变得更差。低温热塑板治疗3级以下儿童弹响拇是一种疗效确切、安全易行的保守治疗方法,适合临床推广应用。而对于分级为3级的儿童弹响拇(即交锁状态),建议行手术治疗。To analyze therapeutic effect of 107 children(115 thumbs)with trigger thumbs using low-temperature thermoplastic splints retrospectively.The therapeutic effect was evaluated according to two different evaluation criteria.The results showed that cured in 80 thumbs(69.6%),improved in 20 thumbs(17.4%),and nonimproved in 15 thumbs(13.0%)among 115 thumbs.The total effective rate(cured and improved)was 87.0%.According to the Jung et al grading standard,the number of thumbs in grade 0 increased after treatment,while the number of thumbs in grade 1 and grade 2 decreased after treatment(P<0.05).None of the thumbs became worse after treatment.It s effective,safe and easy to treat trigger thumbs in children graded below 3 with low-temperature thermoplastic splints,and suitable for widespread clinical application.We suggest trigger thumbs in children graded as 3(locking)should be considered for surgical release.
