腹腔镜下急性胃十二指肠穿孔修补术对胃肠道功能的影响

摘　　要：目的分析腹腔镜下急性胃十二指肠穿孔修补术对胃肠道功能的影响。方法选择2016年5月~2019年5月在我院诊治的急性胃十二指肠穿孔患者90例,采用随机数字表法分为对照组和观察组,各45例。对照组采用传统开腹急性胃十二指肠穿孔修补术,观察组采用腔镜下急性胃十二指肠穿孔修补术。比较两组手术时间、术后VAS评分、肛门排气时间、肠鸣音恢复时间、胃蛋白酶(PGⅠ、PGⅡ)水平、胃肠道激素(GAS、VIP、MOT)水平以及并发症发生率。结果观察组手术时间、术后VAS评分、肛门排气时间、肠鸣音恢复时间均优于对照组,差异有统计学意义(P<0.05);治疗后两组PGⅠ、PGⅡ、GAS、VIP、MOT水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为2.22%,低于对照组的22.22%,差异有统计学意义(P<0.05)。结论腹腔镜下急性胃十二指肠穿孔修补术患者创伤小,术后胃肠功能恢复快,且并发症少,利于患者的术后恢复。Objective To analyze the effect of laparoscopic acute gastroduodenal perforation repair on gastrointestinal function.Methods 90 patients with acute gastroduodenal perforation who were treated in our hospital from May 2016 to May 2019 were randomly divided into the control group and the observation group,with 45 cases in each group.The control group underwent conventional open abdominal gastroduodenal perforation repair,and the observation group underwent endoscopic acute gastroduodenal perforation repair.The operation time,postoperative VAS score,anal exhaust time,recovery time of bowel sound,pepsin(PGⅠ、PGⅡ)level,gastrointestinal hormone(GAS,VIP,MOT)level and complication rate were compared between the two groups.Results The operation time,postoperative VAS score,anal exhaust time,and recovery time of bowel sounds in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05).After treatment,the two groups were PGⅠ,PGⅡ,GAS,VIP,and MOT.The water level was higher than that before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 2.22%,which was lower than that in the control group 22.22%,the difference was statistically significant(P<0.05).Conclusion Laparoscopic acute gastroduodenal perforation repair has less trauma,rapid recovery of gastrointestinal function,and less complications,which is beneficial to postoperative recovery.
