早期子宫内膜癌MRI表现特点及临床应用价值分析

摘　　要：目的:分析探讨早期(Ⅰ~Ⅱ期)子宫内膜癌MRI表现特点及对临床的应用价值。方法:回顾性分析我院62例经手术及病理证实的子宫内膜癌患者MR表现,分析探讨各序列的表现特点、敏感性及临床应用价值。结果:其中62例患者中Ⅰa期15例,Ⅰb期25例;Ⅱ期22例。肿瘤T1WI呈等信号,与正常子宫内膜分界不清,T2WI一般表现为中等或高信号,但通常信号混杂,甚至可以是低信号,DWI为明显高信号,增强后表现为轻、中度强化。T2WI分期的敏感度为89.3%,特异度为91.0%,T2WI+DWI分期的敏感度为94.3%,特异度为93.5%。结论:M R常规检查在确定早期子宫内膜癌的范围、分期和判断肿瘤细胞分化程度上具有重要的临床应用价值。Objective To analyze the early endometrial carcinoma(Ⅰ-Ⅱ period)MRI performance characteristics and the clinical application value.Methods The MR manifestations of 62 patients with endometrial carcinoma confirmed by surgery and pathology in our hospital were retrospectively analyzed,and the characteristics,sensitivity and clinical application value of each sequence were analyzed.Results Of 62 patients Ia period of 15 cases,Ib 25 cases;Stage Ⅱ:22 cases.T1WI of tumor presented equal signal,and the boundary with normal endometrium was not clear.T2WI generally presented medium or high signal,but usually mixed signals,or even low signal.DWI presented significantly high signal,and mild or moderate enhancement after enhancement.The sensitivity and specificity of T2WI staging were 89.3%,91.0%,94.3%and 93.5%respectively.Conclusion MR routine examination has important clinical value in determining the scope,stage and differentiation degree of early endometrial carcinoma.
