开窍利咽四步针刺法配合导管球囊扩张术治疗卒中后环咽肌失弛缓型吞咽障碍临床研究

摘　　要：目的观察开窍利咽四步针刺法配合导管球囊扩张术治疗卒中后环咽肌失弛缓型吞咽障碍的临床疗效。方法将60例卒中后环咽肌失弛缓型吞咽障碍患者随机分为观察组和对照组,每组30例。两组均接受常规康复训练,观察组采用开窍利咽四步针刺法配合导管球囊扩张术治疗,对照组仅采用单纯导管球囊扩张术治疗。观察两组治疗前后洼田饮水试验(WST)评分及标准吞咽功能评估量表(SSA)评分的变化。结果两组治疗前后WST评分和SSA评分比较,差异有统计学意义(P<0.05)。观察组治疗后WST评分和SSA评分优于对照组(P<0.05)。结论开窍利咽四步针刺法配合导管球囊扩张术是一种治疗卒中后环咽肌失弛缓型吞咽障碍的有效方法。Objective To observe the clinical efficacy of four-step Kaiqiao Liyan(opening orifices and relaxing throat)needling method plus balloon dilation in treating deglutition disorders due to achalasia of cricopharyngeal muscle after stroke.Method Sixty patients with deglutition disorders due to achalasia of cricopharyngeal muscle after stroke were randomized into an observation group and a control group,with 30 cases in each group.The two groups both received conventional rehabilitation training.In addition,the observation group was intervened by four-step Kaiqiao Liyan needling method plus balloon dilation,while the control group only received balloon dilation.The two groups were evaluated by water swallow test(WST)and standardized swallowing assessment(SSA)before and after treatment.Result The WST and SSA scores changed significantly after intervention in the two groups(P<0.05).After treatment,the WST and SSA scores in the observation group were better than those in the control group(P<0.05).Conclusion Four-step Kaiqiao Liyan needling method plus balloon dilation is an effective approach for deglutition disorders due to achalasia of cricopharyngeal muscle after cerebral stroke.
