针灸治疗痉挛性偏瘫临床选穴规律探析

摘　　要：目的运用数据挖掘技术探索针灸治疗痉挛性偏瘫的临床应用规律。方法运用《中医针灸信息库》、中国期刊全文数据库、重庆维普数据库、万方学术期刊全文数据库对1954年至2018年针灸治疗痉挛性偏瘫的临床文献进行检索,应用《中医针灸信息库》内相关分析软件进行计量分析,SPSS23.0进行聚类分析,Clementine12.0进行关联规则分析。结果应用频次最高的穴位依次为合谷、曲池、肩髃、外关、足三里、阳陵泉;应用频次最高的经络为手阳明大肠经和足阳明胃经;应用频次最高的治疗方法是针刺和针康结合;关联规则分析发现支持度最高的关联群为阳陵泉、足三里、外关、肩髃、曲池和合谷,此6穴均为拮抗肌附着部穴位;聚类分析结果显示穴组可分为3类,合谷、曲池、外关、手三里、肩髃、足三里、阳陵泉、太冲、解溪归为第一类,此9穴均为拮抗肌附着部穴位;极泉、尺泽、内关、三阴交、委中归为第二类,太溪单独归为第三类。结论针灸治疗痉挛性偏瘫以阳经选穴为主,以拮抗肌选穴为多,治疗方法上以针刺、针康结合治疗为主。Objective To analyze the acupoint selection rules in acupuncture-moxibustion treatment of spastic hemiplegia using data mining technique.Method Clinical studies on acupuncture-moxibustion treatment of spastic hemiplegia were retrieved from the Chinese Medicine Acupuncture-moxibustion Information Database,China National Knowledge Infrastructure,Chongqing VIP Database and Wanfang Academic Journal Full-text Database during 1954 and 2018.Quantitative analysis was run by the built-in software of the Chinese Medicine Acupuncture-moxibustion Information Database;clustering analysis was performed using SPSS version 23.0;Clementine 12.0 was used for association rule analysis.Result The most frequently used acupoints were Hegu(LI4),Quchi(LI11),Jianyu(LI15),Waiguan(TE5),Zusanli(ST36)and Yanglingquan(GB34);the most frequently used meridians were Large Intestine Meridian of Hand Yangming and Stomach Meridian of Foot Yangming;the most frequently used methods were acupuncture and acupuncture plus rehabilitation.The association rule analysis found that the acupoint group with the highest support consisted of Yanglingquan(GB34),Zusanli(ST36),Waiguan(TE5),Jianyu(LI15),Quchi(LI11)and Hegu(LI4),and these 6 acupoints are all located around the antagonistic muscles.the clustering analysis showed that the acupoint groups could be divided into three categories:Hegu(LI4),Quchi(LI11),Waiguan(TE5),Shousanli(LI10),Jianyu(LI15),Zusanli(ST36),Yanglingquan(GB34),Taichong(LR3)and Jiexi(ST41)as the first group and these 9 acupoints are located around the antagonistic muscles;the second group included Jiquan(HT1),Chize(LU5),Neiguan(PC6),Sanyinjiao(SP6)and Weizhong(BL40);the third group consisted of Taixi(KI3)alone.Conclusion In acupuncture-moxibustion treatment of spastic hemiplegia,acupoints from yang meridians are mainly selected,predominated by acupoints around the antagonistic muscles,with acupuncture and acupuncture plus rehabilitation as the major treatment methods.
