湿敷法早期干预对降低会阴部放射性皮肤损伤的应用价值探讨

摘　　要：目的探讨康复新湿敷法配合射线保护剂外喷早期干预对会阴部放射性皮肤损伤的防治效果。方法将48例接受放疗的宫颈癌患者按住院号随机数字法分为观察组与对照组。每组各24例,两组均积极进行健康教育,加强心理护理、营养支持和皮肤护理,观察组采用康复新湿敷和射线保护剂外喷局部交替进行,对照组单纯使用射线保护剂外喷局部,比较两组急性粘膜损伤发生时间、损伤程度。结果观察组急性放射性外阴粘膜损伤出现时间显著迟于对照组(P<0.05);观察组分别为12.5%(1~10D)、16.7%(11~20D)、41.6%(21~30D)、29.2%(>30D),对照组分别为41.6%、29.2%、16.7%、12.5%;观察组Ⅰ、Ⅱ级粘膜损伤的例数与Ⅲ、Ⅳ级粘膜损伤的例数显著低于对照组(P<0.05)。结论康复新湿敷法配合射线保护剂外喷早期干预能有效延迟放疗所致的Ⅰ、Ⅱ级外阴粘膜反应发生时间,减少Ⅲ、Ⅳ级粘膜损伤发生例数,降低严重程度,减轻患者痛苦,提高生活质量,保证放疗按期完成。Objective To investigate the prevention and treatment effect of rehabilitation new wet compress meth od combined with radiation protection agent for early intervention on radioactive skin injury of perineum.Meth od Forty-eight patients with cervical cancer who received radiotherapy were divided into observation group and control group according to the hospital number random number method.24 cases in each group,both groups actively carried out health education,strengthen psychological care,nutritional support and skin care,the observation group was treated with rehabilitation new wet compress and radiation protection agent external spray alternately,the control group simply used radiation protectant spray Locally,the time and degree of injury of acute mucosal injury were compared between the two groups.Results The time of acute radiation vulvar muco sal injury in the observation group was significantly later than that in the control group(P<0.05).The observa tion group was 12.5%(1~10D),16.7%(11~20D),41.6%(21~30D),29.2%(>30D),41.6%,29.2%,16.7%,12.5%in the control group;the number of cases of mucosal injury in grade I and II and the number of mucosal lesions in grade III and IV in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Rehabilitation new wet compress combined with radioprotectant external spray can effective ly delay the occurrence of grade I and II vulvar mucosal reaction caused by radiotherapy,reduce the number of cases of grade III and IV mucosal injury,reduce the severity and reduce the pain of patients.Improve the quality of life and ensure that radiotherapy is completed on schedule.
