摘 要:目的观察2型糖尿病(type 2 diabetes mellitus,T2DM)患者外周血单个核细胞(peripheral blood mononuclear cell,PBMC)NLRP3炎症小体活性与冠状动脉粥样硬化病变程度的相关性。方法200名新诊断的未使用任何降糖药物的T2DM患者据冠脉造影结果分为非冠心病组(20例),单支病变组(60例),双支病变组(60例),多支病变组(60例)。比较各组患者PBMC NLRP3 mRNA、蛋白及血浆IL-1β表达变化,并分析血浆IL-1β水平与冠状动脉粥样硬化病变Gensini评分的相关性。结果与非冠心病组相比,冠心病组患者PBMC NLRP3 mRNA、蛋白及血浆IL-1β表达水平显著升高(P<0.01),而双支及多支病变组冠心病患者较单支病变组患者NLRP3 mRNA、蛋白及血浆IL-1β表达水平增强(P<0.05);T2DM患者血浆IL-1β水平与冠状动脉Gensini评分呈正相关(r=0.351 6,P<0.001)。结论NLRP3炎症小体活性与糖尿病患者冠状动脉粥样硬化病变程度密切相关,NLRP3炎症小体可能成为糖尿病患者动脉粥样硬化病变的有效干预靶点。Objective To observe the correlation between NLRP3 inflammasome in peripheral blood mononuclear cell(PBMC)and plasma IL-1βexpression in patients with type 2 diabetes mellitus(T2DM)and the severity of coronary artery stenosis.Methods A total of 200 newly diagnosed T2DM patients without any medication were divided into non-coronary heart disease group(20 cases),single-vessel group(60 cases),double-vessel group(60 cases)and multi-vessel group(60 cases).Changes of NLRP3 mRNA,protein in PBMC and plasma IL-1βwere observed in four groups.The correlation between IL-1βand Gensini score was analyzed.Results Compared with non-coronary heart disease patients,the levels of NLRP3 mRNA,protein in PBMC and IL-1βin plasma were significantly increased in patients with DM and coronary heart disease(P<0.01),and the levels were significantly higher in two-vessel or multi-vessel groups than in single-vessel group(P<0.05).The level of IL-1βwas significantly positively correlated with Gensini score(r=0.351 6,P<0.001).Conclusion NLRP3 inflammasome activation is significantly associated with the severity of coronary stenosis in diabetic patients and has potential therapeutic implications for diabetic cardiovascular complications.
