杵针在脑卒中偏瘫患者肢体功能康复中的效果研究

摘　　要：目的探讨杵针疗法在脑卒中伴有肢体偏瘫患者中应用方法和效果。方法我院228例脑卒中住院患者,按随机数字表法分为杵针组、针灸组、杵针联合针灸组各76例,针灸组选用最适宜的穴位进行针灸治疗;杵针组选用八阵穴、河车路、特定穴位等进行杵针治疗;杵针联合针灸组采用杵针和针灸治疗。干预前和干预4周后评价3组患者的抑郁情绪、疼痛、日常生活能力、运动功能和神经功能缺损情况。结果干预后3组患者的日常生活能力、运动及神经功能、抑郁情绪均优于干预前(P<0.05);杵针联合针灸组的运动功能优于杵针组和针灸组,日常生活能力、抑郁评分优于针灸组(P<0.05);杵针组疼痛评分低于针灸组和杵针联合针灸组(P<0.05)。结论杵针联合针灸能有效提高患者的运动功能、日常生活能力,促进神经功能恢复,缓解抑郁情绪,对于不能耐受针灸疼痛的患者可以使用杵针促进患者肢体功能康复。Objective To investigate the method and effect of pestle needle therapy in the treatment of stroke patients with hemiplegia.Methods Two hundred and twenty two inpatients with stroke in our hospital were randomly divided into clubbing pestle needle therapy group(n=76),acupuncture and moxibustion therapy group(n=76)and pestle needle combined with acupuncture group(n=76).The acupuncture patients used the most suitable acupuncture points for acupuncture.The pestle needle group used Ba zhen points and Hechelu points as main points as well as selected points in whole body as adjuvant points of treatment.The pestle needle combined with acupuncture group was treated with the two methods as described above.The daily living ability,motor function,neurolog ical deficit,pain and depression of the three groups were evaluated before and after 4 weeks of intervention.Results After intervention,the activities of daily living,motor function,neurological deficit and depression were better than those before intervention(P<0.05).The motor function of the pestle needle combined with acupuncture group were better than that of the acupuncture group and pestle nee dle group(P<0.05)and the ability of life and depression score was better than that of acupuncture group(P<0.05).The pain score of the pestle needle group was lower than that of the acupuncture group and the pestle needle combined acupuncture group(P<0.05).Conclusion Pestle needle combined with acupuncture therapy can effectively improve the motor function and daily living ability of the patients,promote the recovery of nerve function,relieve the depression and reduce the pain during operation.For patients who cannot tolerate acupuncture pain,pestle needle can be used to promote the recovery of limb function.
