ICU患者谵妄评估约束分级方案在神经外科意识障碍患者中的应用

摘　　要：目的讨论ICU患者谵妄评估约束分级方案在神经外科意识障碍患者约束评估管理中的应用效果观察。方法2019年3~8月我科收治的意识障碍患者43例,按照随机数字表法分为对照组21例和试验组22例。试验组采用ICU患者谵妄评估约束分级方案,对照组采用格拉斯哥分级和RASS镇静程度评估表。比较两组患者约束强度、约束率及患者满意度。结果试验组约束例数和烦躁增加例数高于对照组(P<0.05),两组约束部位皮损例数及患者满意度分数差异无统计学意义(P>0.05)。结论ICU患者谵妄评估约束分级方案能更好地为评估人员提供清晰具体的约束分级方案,可明显减少患者约束率。适合在神经外科意识障碍患者的约束评估中运用和推广。Objective To discuss the applicability of the ICU delirium evaluation and restraint classification scheme in the management of neurosurgical patients with consciousness disorders.Methods From March 2019 to August 2019,43 patients with con sciousness disorders admitted to our department were randomly divided into control group(n=21)and experimental group(n=22).The experimental group adopted the ICU patients delirium evaluation and control group adopted the Glasgow classification and RASS sedation evaluation table.The accuracy,effectiveness,physical restraint rate and satisfaction were compared between the two groups.Re sults The number of cases of restraint and increase of irritability in the experimental group was higher than that those in the control group(P<0.05).There was no significant difference in the number of skin lesions at the restraint site and the patient satisfaction score between the two groups.Conclusion ICU delirium evaluation and restraint classification scheme can better provide a clear and specific constraint grading scheme for constraint evaluators and significantly reduce the patient constraint rate.It is suitable for the application and promotion of constraint assessment in patients with consciousness disorders in neurosurgery.
