厦门市某三甲医院2013-2018年手足口病疫情分析

摘　　要：目的分析厦门市某三甲医院手足口病疫情特征,为相关部门制定防控措施提供依据。方法对厦门市某三甲医院2013年1月-2018年12月通过中国疾病预防控制信息系统进行网络直报的手足口病个案进行流行病学特征和报告质量分析。结果厦门市某三甲医院2013-2018年手足口病发病报告数、重症数和死亡数分别为4058例、16例和2例,分别占比厦门市医疗卫生机构报告数、重症数和死亡数的8.01%、27.12%和50.00%。发病数主要分布在5月、6月和9月。男女比为1.66:1;男性和女性均为1~岁组发病数居前,分别为850例和565例,占比分别为33.56%和37.05%。区县分布主要为集美(2434例)、思明(523例)等地。实验室检测数呈上升趋势(F=17.795,P=0.014),2例死亡病例均为EV71病原型。报告及时率为99.90%,报告时间准确率为99.88%;有效证件填写完整率呈上升趋势(F=23.945,P=0.008)。结论该院是厦门市手足口病患者诊疗的主要医院之一,特别是病情危重患者;EV71病原型易引起重症甚至死亡;报告质量应进一步提高;相关部门可以根据以上信息以及结合三间分布特征和实验室检测能力的提高等,针对性地制定防控措施。Objective Analyze the characteristics of hand,foot and mouth disease(HFMD)epidemics in a Grade-A Tertiary hospital in Xiamen,and provide evidence for relevant departments to develop prevention and control measures.Methods The epidemiological characteristics and report quality of HFMD cases reported directly by the Grade-A Tertiary hospital in Xiamen City through the China Disease Prevention and Control Information System Network during January 2013 to December 2018 were analyzed.Results The number of reported cases,severe cases and deaths of HFMD in the Grade-A Tertiary hospital in Xiamen City in 2013-2018 were 4058 cases,16 cases and 2 cases,respectively,accounting for 8.01%,27.12%and 50.00%of the reports of Xiamen medical and health institutions.The incidence was mainly distributed in Jun,May and Sep.The male-female ratio was 1.66:1;both men and women were in the first group with 850 cases and 565 cases,accounting for 33.56%and 37.05%,respectively.The districts were mainly distributed in Jimei(2434 cases)and Siming(523 cases).The number of laboratory tests showed an upward trend(F=17.795,P=0.014),and the two death cases were all EV71 type.The report timely rate was 99.90%,and the reporting time accuracy rate was 99.88%,and the effective document completion rate showed an upward trend(F=23.945,P=0.008).Conclusion The hospital is one of the main hospitals for diagnosis and treatment of HFMD patients in Xiamen,especially critically ill patients.The prototype of EV71 disease is easy to cause serious illness or even death.The quality of report should be improved.So,relevant departments can formulate prevention and control measures according to the above information and the combination of the three distribution characteristics and the improvement of laboratory testing capabilities.
