孟鲁司特钠联合曲安奈德鼻喷雾剂治疗变应性鼻炎的疗效及对患者血清黏附分子和Th1/Th2细胞因子的影响

摘　　要：目的探讨孟鲁司特钠联合曲安奈德鼻喷雾剂运用于变应性鼻炎患者中的价值及对其血清黏附分子和Th1/Th2细胞因子的影响。方法资料收集该院106例变应性鼻炎患者,入院时间均为2016年1月至2018年6月,按照随机数字表法将其分为对照组(给予曲安奈德鼻喷雾剂治疗)与联合组(在前者基础上同时应用孟鲁司特钠治疗)各53例,对比两组患者治疗有效率、局部和全身症状,并对比治疗前后血清黏附分子、Th1/Th2细胞因子水平变化。结果治疗后联合组有效率比对照组明显更高,差异有统计学意义(P<0.05);治疗前,两组患者的临床症状对比差异均无统计学意义(P>0.05),治疗后,联合组鼻部症状及相关症状较干预前及对照组同期有明显好转,差异有统计学意义(P<0.05);治疗前两组患者血清可溶性细胞间黏附分子(sICAM-1)与P-选择素水平对比差异无统计学意义(P>0.05),治疗后两组sICAM-1与P-选择素水平均较治疗前有所降低,但联合组各指标下降幅度明显大于对照组,差异有统计学意义(P<0.05);治疗前后两组Th1/Th2细胞因子水平对比差异无统计学意义(P>0.05),治疗后两组IL-2、IFN-γ、IL-10水平明显高于治疗前,血清IL-4均低于治疗前,组间对比发现,联合组各指标下降/上升幅度均大于对照组,差异有统计学意义(P<0.05)。结论孟鲁司特钠联合曲安奈德鼻喷雾剂治疗变应性鼻炎效果显著,能有效改善其血清因子水平,促进患者早日康复,值得在临床推广使用。Objective To study the application value of montelukast sodium combined with Triamcinolone Acetonide Nasal Spray in allergic rhinitis and its impact on serum adhesion molecules and Th1/Th2 cytokines.Methods The clinical data in 106 cases of allergic rhinitis treated in this hospital from January 2016 to June 2018 were collected;the cases were divided into the control group(giving Triamcinolone Acetonide Nasal Spray therapy)and combined group(simultaneously using montelukast sodium on the formers basis)according to the random number table method,53 cases in each group.The effective rate and local and general symptoms were compared between the two groups,and the changes of serum adhesion molecules and Th1/Th2 cytokines levels were compared between before and after treatment.Results After treatment,the effective rate in the combined group was significantly higher than that in the control group with statistical difference(P<0.05);before treatment,the clinical symptoms had no statistically significant difference between the two groups(P>0.05);after treatment,the nasal symptoms and relevant symptoms in the combined group were significantly better than those before intervention and those in the control group,and the differences were statistically significant(P<0.05);before treatment,serum soluble intercellar adhesion molecule-1(sICAM-1)and P-selectin level had no statistically significant difference between the groups(P>0.05);but the decrease range of various indicators in the combined group was significantly greater than that in the control group with statistical difference(P<0.05);before and after treatment,the Th1/Th2 cytokine level had no statistically significant difference between the two groups(P>0.05);after treatment,interleukin-2(IL-2),interferon-γ(IFN-γ)and interleukin-10(IL-10)levels in the two groups were higher than those before treatment;serum IL-4 level was lower than that before treatment;but the inter-group comparison found that the declining/increasing ranges of various indicators in the combined gro
