摘 要:目的探究组织多普勒超声成像系统(tissue Doppler imaging,TDI)与脉冲多普勒超声心动图(pulse Dolpper,PD)对胎儿心脏传导时间的诊断价值,为临床诊断提供参考。方法选取2017年1月-2019年1月在我院受检的210例孕妇作为研究对象,应用TDI与PD测量胎儿的房室传导时间(atrio-ventricular conduction time intervals,AV)以及心室开始收缩至下一个心动周期心房开始收缩的时间间期(ventriculo-atrial conduction time intervals,VA)。结果 (1)在TDI与PD的联合检测下,受检胎儿的心脏结构均正常,偶发性心律失常共18例,占8.57%。(2) TDI测量显示,胎儿总体AV平均值为(128.78±11.63)ms,VA平均值为(281.37±21.29)ms;PD测量显示胎儿总体AV平均值为(127.63±12.01)ms,VA平均值为(282.50±22.67)ms,不同方法下的AV、VA比较差异均无统计学意义(P>0.05)。(3)以组织多普勒测量结果为标准,显示AV值与孕期呈正相关(r=0.778,P<0.001),与胎儿心率水平呈负相关(r=-0.806,P<0.001);同时VA值与孕期呈正相关(r=0.784,P<0.001),与胎儿心率水平呈负相关(r=-0.817,P<0.001)。结论 TDI与PD成像系统均能有效地测定出胎儿的心脏传导时间,为医护人员有效监测和评估胎儿的心脏活动与异常提供参考依据。Objective To explore the clinical diagnostic value of tissue Doppler imaging system(TDI) and pulsed Doppler ultrasound imaging system(PD) for fetal heart conduction time.Methods Two hundred and ten pregnant women examined in Tangshan Maternal and Child Health Hospital from January 2017 to January 2019 were selected as subjects.The fetal atrio-ventricular conduction time intervals(AV) and the time intervals from the onset of ventricular contraction to the onset of atrial contraction(VA) of the next cardiac cycle were measured by TDI and PD system.Results ①Under combined detection of TDI and PD,the fetal heart structure was normal.There were 18 cases of occasional arrhythmia,accounting for 8.57%.②TDI measurements showed that average AV of the fetus was(128.78±11.63) ms,and average VA was(281.37±21.29) ms;PD measurements showed that average AV of the fetus was(127.63 ±12.01) ms,and average VA was(282.50 ±22.67) ms.There was no significant difference in AV and VA between different methods(P>0.05).③According to tissue Doppler measurements,AV was positively correlated with pregnancy(r=0.778,P<0.001),negatively correlated with fetal heart rate(r=-0.806,P<0.001),and VA was positively correlated with pregnancy(r=0.784,P<0.001),negatively correlated with fetal heart rate(r=-0.817,P<0.001).Conclusion TDI and PD imaging systems can effectively measure fetal heart conduction time,and help medical staff effectively monitor and evaluate fetal heart activity and abnormalities.
