丹参川芎注射液联合纳络酮注射液治疗COPD合并呼吸衰竭的临床研究

摘　　要：目的探讨丹参川芎注射液联合纳络酮注射液治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床研究。方法将96例COPD合并呼吸衰竭患者分为观察组和对照组。对照组采用纳络酮注射液治疗。观察组在对照组基础上,采用丹参川芎注射治疗。检测患者血气指标[动脉血氧分压(PaO2)、动脉二氧化碳分压(PaCO2)、酸碱值(pH)、动脉血氧饱和度(SaO2)]和肺功能指标[1秒用力呼气容积(FEV1)、FEV1/肺活量(FVC)、最大呼气流量(PEF)]的水平。检测患者治疗前后C反应蛋白(CRP)、白细胞介素6(IL-6)、单核细胞趋化蛋白1(MCP-1)的水平。结果观察组的总有效率比对照组高,差异有统计学意义(P<0.05);二组治疗后的PaO2、pH、SaO2明显升高,PaCO2明显降低(P<0.05);观察组治疗后的PaO2、pH、SaO2比对照组高,PaCO2比对照组低,差异有统计学意义(P<0.05);二组治疗后的FEV1、FEV1/FVC、PEF明显升高(P<0.05);观察组治疗后的FEV1、FEV1/FVC、PEF比对照组高,差异有统计学意义(P<0.05);二组治疗后的CRP、IL-6、MCP-1明显降低(P<0.05);观察组治疗后的CRP、IL-6、MCP-1比对照组低,差异有统计学意义(P<0.05)。二组治疗过程中无明显不良反应发生。结论丹参川芎注射液可提高COPD合并呼吸衰竭的疗效,改善血气指标,提高肺功能,减轻炎症反应。Objective To investigate the clinical study of Danshen Chuanxiong injection combined with Naloxone injection in the treatment of COPD with respiratory failure.Methods Ninety-six COPD patients with respiratory failure were divided into observation group and control group.The control group was treated with naloxone injection.The observation group was treated with Salvia miltiorrhiza injection and Chuanxiong injection on the basis of the control group.The levels of blood gas indexes(PaO2,PaCO2,pH,SaO2) and pulmonary function indexes(FEV1,FEV1/FVC,PEF) were measured.The levels of C-reactive protein(CRP),interleukin-6(IL-6) and monocyte chemoattractant protein-1(MCP-1) were measured before and after treatment.Results The total effective rate of the observation group was higher than that of the control group.The difference was statistically significant(P<0.05).After treatment,PaO2,PH and SaO2 increased significantly and PaCO2 decreased significantly in both groups(P<0.05).After treatment,PaO2,PH and SaO2 in the observation group were higher than those in the control group,and PaCO2 was lower than those in the control group.The difference was significant(P<0.05).After treatment,FEV1,FEV1/FVC and PEF in the two groups increased significantly(P<0.05).The FEV1,FEV1/FVC and PEF in the observation group were higher than those in the control group after treatment.The difference was significant(P<0.05).CRP,IL-6 and MCP-1 in the two groups were significantly decreased after treatment(P<0.05).After treatment,CRP,IL-6 and MCP-1 in the observation group were lower than those in the control group.The difference was significant(P<0.05).There were no obvious adverse reactions during the treatment of the two groups.Conclusion Danshen Chuanxiong injection can improve the curative effect of COPD with respiratory failure,improve blood gas index and lung function,and alleviate inflammation.
