重症肢体缺血的缺血性溃疡患者足部动脉弓病变的影响因素分析

摘　　要：目的分析重症肢体缺血的缺血性溃疡患者足部动脉弓病变的影响因素。方法择取该院2017年3月-2018年9月期间收治的75例重症肢体缺血的缺血性溃疡患者作为研究对象,收集患者年龄、过往病史以及各项生化指标检测数据,根据足部动脉弓血管造影结果将患者分为参考组(足部动脉弓良好,18例)与探究照组(足部动脉弓病变,57例),利用单因素分析和多因素分析探究影响足部动脉弓病变的影响因素。结果根据单因素分析结果可知,糖尿病病程不低于10年、糖化血红蛋白水平(FBG)升高以及平均血小板体积(MPV)升高均是影响足部动脉病变的影响因素;使用logistic回顾分析发现糖尿病病程≥10年、平均血小板体质以及糖化血红蛋白数值均属于独立危险因素。结论利用糖尿病病程、糖化血红蛋白浓度以及平均血小板体积可有效预测重症肢体却行的缺血性溃疡患者足部动脉弓病变风险。Objective To analyze the influencing factors of foot arterial arch lesion in ischemic ulcer patients with severe limb ischemia. Methods Will pick our hospital during March 2017 to September 2018 were treated 75 cases of patients with critical limb ischemia of ischemic ulcers as the research object,collect patients’ age,past history and the biochemical indexes detection data, according to the result of the foot arch angiography divided the patients into reference groups(the foot arch is good,18 people) and explore according to group(foot arch lesions,57 people),using the single factor analysis and multiple factor analysis to explore the influence factors of affect the foot arch lesions. Results According to the results of univariate analysis,the duration of diabetes was not less than 10 years,the increase of glycosylated hemoglobin(FBG) level and the increase of mean platelet volume(MPV) were all the influencing factors of foot arterial diseases.Logistic retrospective analysis showed that the duration of diabetes ≥10 years,average platelet constitution and hba1 c were all independent risk factors. Conclusion Diabetic course glycosylated hemoglobin concentration and mean platelet volume were used to effectively predict the risk of arterial arch lesions in feet in patients with ischemic ulcer in severe limbs.
