某院2016年至2018年门急诊麻醉药品用药分析

摘　　要：目的促进麻醉药品的合理使用。方法收集某院西区门急诊2016年至2018年麻醉药品使用患者基本情况、药品品种、药品用量、销售金额、用药频度(DDDs)、药物利用指数(DUI),并分析不合理处方。结果该院麻醉药品的使用以老年(≥60岁)患者居多,以肿瘤患者为主(占93%以上),最常见肿瘤类型为消化道肿瘤(51.97%),病种分布最多的为肺癌(24.66%);常用药品剂型为片剂、贴剂和注射剂;吗啡缓释片(规格为30 mg)、羟考酮缓释片(规格为10 mg)的DDDs位居前列,羟考酮缓释片(规格为10 mg)用量呈逐年上升趋势,吗啡缓释片(规格为30 mg)用量呈逐年下降趋势;处方不合理原因主要集中于前记书写不完整、用法用量不适宜。结论该院麻醉药品的品种、剂型、给药途径、DUI值基本符合规范,但仍有不合理处方,需持续改进。Objective To promote the rational use of narcotic drugs.Methods The basic information,drug varieties,drug dosage,sales amount,drug use frequency(DDDs)and drug utilization index(DUI)of the patients used narcotic drugs in the outpatient and emergency department of the western branch of a hospital from 2016 to 2018 were collected,and the unreasonable prescriptions were analyzed.Results The elderly patients(≥60 years old)were the majority in the use of narcotic drugs,most of them were patients with tumor(more than 93%),the most common type of tumor was digestive tract tumor(51.97%),and the most common type of disease was lung cancer(24.66%).The dosage forms of commonly used drugs were tablets,patches and injections,the DDDs of Morphine Sustained-Release Tablets(30 mg)and Oxycodone Sustained-Release Tablets(10 mg)ranked the top,the dosage of Oxycodone Sustained-Release Tablets(10 mg)increased year by year,while that of Morphine Sustained-Release Tablets(30 mg)decreased year by year.The unreasonable prescriptions mainly included incomplete writing of prescriptive notes and improper usage and dosage.Conclusion The variety,dosage form,route of administration and DUI value of narcotic drugs in this hospital conform to the norms,but there are still unreasonable prescriptions,which need to be continuously improved.
