摘　　要：目的探讨血浆血管性血友病因子(vWF)对不稳定型心绞痛(UAP)的预测价值。方法测定90例UAP和75例稳定型心绞痛(SAP)患者治疗前vWF水平(vWF:A%)、凝血因子VⅢ活性(FVⅢ∶C)、白细胞(WBC)和超敏C-反应蛋白(hs-CRP)水平,以Pearson相关性分析、Logistic回归和ROC曲线分别评估各指标的相关性、UAP危险因素以及危险指标对UAP的预测效能。结果UAP组vWF∶Ag、FVⅢ∶C、WBC和hs-CRP水平均高于SAP组(P<0.001);vWF∶Ag水平与FVⅢ∶C和hs-CRP水平均呈高度正相关(r值分别为0.701和0.728,P<0.001)。以SAP和UAP分组,多因素分析显示vWF、FVⅢ和hs-CRP均为UAP的独立危险因素(OR值分别为5.215、4.401和3.235,P<0.001)。ROC曲线分析显示,vWF预测UAP的曲线下面积(AUC)为0.853,高于FVⅢ和hs-CRP(0.802和0.814);在cut-off值为116%时,vWF预测UAP的敏感度和特异度分别为83%和71%。结论vWF水平升高是不稳定型心绞痛的独立风险因子,对其发生有较高的预测价值。Objective To evaluate the value of von willebrand factor(vWF)in predictive plasma unstable angina pectoris(UAP).Methods The levels of vWF,FVⅢ∶C,WBC and high-sensitive C reactive protein(hs-CRP)in 90 patients with UAP and 75 patients with stable angina pectoris(SAP)were measured before treatment.Pearson correlation analysis,logistic regression analysis and ROC curve analysis were used to evaluate the correlation between the parameters,UAP risk factors and the predictive power of risk factors for UAP.Results Levels of vWF,FVⅢ∶C,WBC and hs-CRP in the UAP group were higher than that in SAP group(P<0.001).There was a positive correlation between vWF level and FVⅢ∶C or hs-CRP level(r^2=0.701,r^2=0.728,P<0.001).Multivariate analysis showed that vWF,FVⅢ∶C and hs-CRP were the independent risk factors for UAP(OR=5.215,OR=4.401 and OR=3.235,P<0.001).ROC curve analysis showed that the area under the curve(AUC)of vWF(0.853,P<0.01)was higher than that of FVⅢ∶C and hs-CRP.Based on the cut-off value(116%)of vWF,the sensitivity was 83%,specificity was 73%for UAP prediction.Conclusion Elevated vWF level is the independent risk factor for unstable angina pectoris,and vWF has a high predictive value for the occurrence of unstable angina pectoris.
