人乳头状瘤病毒基因型与尖锐湿疣复发和患者细胞免疫功能的关系

摘　　要：目的探讨人乳头状瘤病毒(HPV)基因型与尖锐湿疣(CA)复发和患者细胞免疫功能的关系。方法选取2015年1月-2017年6月在本院治疗的CA患者80例,同时选取健康志愿者50例作为对照组,检测CA患者疣体HPV基因型,同时检测CA患者和对照组T淋巴细胞亚群。结果80例CA患者经治疗后,复发患者28例,未复发患者52例;复发患者混合感染和高危型感染比例分别为46.43%和32.14%,明显高于未复发患者(P<0.05);复发患者CD4^+水平明显低于未复发患者和对照组(P<0.05),而CD8^+明显高于未复发患者和对照组(P<0.05);未复发患者CD4^+水平明显低于对照组(P<0.05),而CD8^+明显高于对照组(P<0.05);复发和未复发患者CD3^+水平均低于对照组,上述差异均有统计学意义(P<0.05);高危型、低危型和混合感染患者CD3^+、CD4^+、CD8^+和NK细胞比较差异无统计学意义(P>0.05)。结论HPV基因型、细胞免疫功能与尖锐湿疣患者复发有关,其中HPV基因型与患者细胞免疫功能无明显关系。Objective To investigate the relationship of human papillomavirus(HPV)genotype with the recurrence of condyloma acuminata(CA)and patients'cellular immune function.Methods Eighty CA patients who were treated in our hospital from January 2015 to June 2017 were selected,and 50 healthy volunteers were selected as the control group,the HPV genotypes of the verrucous body of CA patients were detected,and the T lymphocyte subsets were detected in CA and control groups.Results Eighty CA patients were treated,with 28 cases recurrent and 52 cases not recurrent;The proportion of mixed infection and high risk infection in recurrent patients were 46.43%and 32.14%,which was significantly higher than that of non recurrent patients(P<0.05);The level of CD4^+in recurrence patients was significantly lower than that in patients without recurrence and control group(P<0.05),while CD8^+was significantly higher than that in patients without recurrence and control group(P<0.05);The CD4^+level of patients without recurrence was significantly lower than that of the control group(P<0.05),while CD8+was significantly higher than that of the control group(P<0.05);The levels of CD3+^in recurrent and non recurrent patients were significantly lower than those in the control group,with the differences statistically significant(P<0.05);There was no statistical significance on the difference in CD3^+,CD4^+,CD8^+and NK cells in patients with high risk,low risk and mixed infection(P>0.05).Conclusion The HPV genotypes and cellular immune functions are related to the recurrence of condyloma acuminata,and there is no significant relationship between the HPV genotype and the cellular immune function of the patients.
