孕中期空腹及餐后两小时血浆胰岛素参考区间的调查

摘　　要：目的调查孕中期空腹及餐后两小时血浆胰岛素参考区间。方法按照中华医学会妇产科学分会产科学组《妊娠合并糖尿病诊治指南(2014)》,纳入307名健康孕中期孕妇,采用罗氏免疫化学发光仪(E-601)测定空腹及餐后两小时血浆胰岛素水平。按照孕周和年龄分组进行比较,并根据分布特征选取适当方法计算生物参考区间。结果孕中期空腹及餐后两小时血浆胰岛素水平呈偏态分布。按孕周分为2组(孕19~22周;孕23~26周),空腹孕周组间(P=0.186)及餐后两小时孕周组间(P=0.627)差异均无统计学意义;按年龄分为2组(21~30岁;31~41岁),空腹年龄组间差异有统计学意义(P=0.049),餐后两小时年龄组间差异无统计学意义(P=0.183)。307名健康孕中期孕妇空腹血浆胰岛素水平中位数为10.24mU/L,2.5%~97.5%范围是3.73~20.16mU/L。餐后两小时血浆胰岛素水平中位数为49.32mU/L,2.5%~97.5%范围是10.21~142.15mU/L。结论孕中期空腹及餐后两小时血浆胰岛素水平较正常女性高,应单独设立参考区间。Objective To investigate the reference ranges of fasting and 2-hour plasma insulin in women in the second trimester.Methods According to the guidelines for the diagnosis and treatment of pregnancy with diabetes mellitus(2014)issued by the Obstetrics Group of the Chinese Medical Association of Gynecology and Obstetrics,plasma insulin levels were determined by using Roche immunosystem e601 in 307 healthy pregnant women in the second trimester.The reference intervals were calculated by using appropriate statistics methods according to the insulin level distribution characteristics.Results The fasting and 2-hourplasma insulin levels showed a skewed distribution in the second trimester of pregnancy.There was no significant difference in fasting and 2-hour postprandial insulin levels among gestational weeks.There were significant differences in fasting insulin level among age groups(P=0.049),but there was no significant difference in 2-hour postprandial insulin level among age groups(P=0.183).The median fasting plasma insulin level was 10.24mU/L in 307 healthy pregnant women during the second trimester of pregnancy,and the range of 2.5%-97.5%was 3.73-20.16mU/L.The median 2-hour plasma insulin level was 49.32mU/L,and the range of 2.5%-97.5%was 10.21-142.15mU/L.Conclusion Plasma insulin levels in pregnant women are higher than those in normal women,and the reference ranges should be established separately.
