摘　　要：目的研究类风湿关节炎患者孕期和产后期糖皮质激素受体基因多态性与疾病活动性的关系。方法通过采用基因分型检测和DAS28疾病活动评分检测方法,分别检测糖皮质激素受体基因的多态性和类风湿关节炎患者孕期和产后期在未使用GC组和使用GC组的疾病活动性。结果Bcl I多态性及9β多态性的杂合子或纯合子携带者分别有93名(63.3%)以及54名(36.7%);GC-S组中的GC治疗患者在产后期间的疾病活动显著低于GC-I中的患者(P<0.05);在未接受GC治疗的患者中,妊娠期或产后期的疾病活动水平和疾病过程似乎不受GR基因型差异的影响(P>0.05)。结论在类风湿关节炎患者中,GR多态性确定的GC敏感性差异会影响GC治疗女性产后期的疾病活动水平。Objective To study the relationship between glucocorticoid receptor gene polymorphism and disease activity in patients with rheumatoid arthritis during pregnancy and postpartum period.Methods The polymorphism of the glucocorticoid receptor gene and the disease activity in the late pregnancy and following postpartum stages of the rheumatoid arthritis patients were detected in the GC-free group and the GC-treated group by using the genotyping test and the DAS28 disease activity score detection method,respectively.Results We found 92(62.6%)and 54(36.7%)heterozygous or homozygous carriers of BclI and 9βpolymorphisms,respectively;For GC-treated patients in the GC-S group during the postpartum period,the disease activity was significantly lower than that in patients with GC-I(P<0.05);Among patients who did not receive GC,the levels of disease activity and disease progression during pregnancy or postpartum stage seemed not be affected by differences in GR genotype(P>0.05).Conclusion In patients with rheumatoid arthritis,the difference in GC sensitivity determined by the GR polymorphism affects the level of disease activity in women with GC during the postpartum period.
