摘  要:目的评价磁敏感加权成像(Susceptibility Weighted Imaging,SWI)及氢质子波谱(1H Magnetic Resonance Spectroscopy,1H-MRS)成像在鉴别各级脑胶质瘤中的诊断价值。方法按照WHO分级方法将81例脑胶质瘤患者分为3组:Ⅱ级30例,Ⅲ级13例,Ⅳ级38例。所有患者均于术前完成SWI及1H-MRS成像,并经过术后组织病理学证实。通过后处理测量SWI及1H-MRS各个参数值,并进行统计学分析。结果rNAA在鉴别Ⅱ级与Ⅲ级中有统计学意义(P<0.05)。r Cho/NAA、r NAA/Cr在鉴别Ⅱ级与Ⅲ级、Ⅱ级与Ⅳ级中有统计学意义(P<0.05)。SWI在各组间均具有显著的统计学差异(P<0.05)。联合两种技术的ROC曲线下面积AUC、敏感性和特异性均大于单独应用某一种技术。结论联合SWI及1H-MRS对脑胶质瘤分级的诊断效能大于单独应用某一种技术;SWI比1H-MRS的诊断效能高。Objective To evaluate the value of susceptibility weighted imaging(SWI)and 1 H magnetic resonance spectroscopy(1 H-MRS)in diagnosis of glioma.Methods According to WHO classification method,81 patients with glioma were divided into three groups:30 cases of grade Ⅱ,13 cases of grade Ⅲ and 38 cases of grade Ⅳ.All patients underwent SWI and 1 H-MRS imaging before operation,and were confirmed by postoperative histopathology.All parameters of SWI and 1 H-MRS were measured by post-processing,and then were analyzed statistically.Results Parameter r NAA had statistical significance in differentiating grade Ⅱ from grade Ⅲ(P<0.05).r Cho/NAA and r NAA/Cr had statistical significance in differentiating grade Ⅱ from grade Ⅲand grade Ⅱ from grade Ⅳ(P<0.05).There were significant statistical differences in SWI among groups(P<0.05).By combining these two techniques,the AUC,sensitivity and specificity were greater than the application of a single technique.Conclusion The diagnostic efficiency of combined SWI and MRS in grading of glioma is higher than that of single technique.SWI has higher diagnostic efficacy than 1H-MRS.
