摘 要:目的探讨NDA倍体分析对南疆维吾尔族妇女宫颈癌早期筛查中的诊断价值。方法对2017年8月-2019年2月本院252例南疆维吾尔族女性患者进行宫颈癌早期筛查,均采用液基细胞学检查(TCT)+细胞DNA倍体定量分析,任一结果阳性的进行宫颈活组织检查,以病理检查结果为金标准,来探讨液基细胞学检查、细胞DNA倍体分析和液基细胞学检查联合细胞DNA倍体分析对早期宫颈癌筛查中的灵敏度和准确度。结果96例患者宫颈活检阳性,其中为42例为CINI,25例为CINII,18例为CINIII,11例为宫颈癌。液基细胞学检查、细胞DNA倍体分析和液基细胞学检查联合细胞DNA倍体分析的阳性率分别为:71.03%,76.19%,92.86%,联合检测的灵敏度最高,P<0.05具有统计学意义。TCT、DNA倍体分析、两者联合的灵敏度分别为:78.13%,87.5%,93.75%,联合检测的灵敏度最高,P<0.05具有统计学意义。结论DNA倍体定量分析联合薄层液基细胞学检查对宫颈癌早期筛查灵敏度高,对于南疆妇女宫颈癌筛查有较高的临床价值。Objective To explore the diagnostic value of NDA ploidy analysis in the early screening of cervical cancer in uygur women in southern of Xinjiang.Methods From August 2017 to February 2019,252 cases of female uygur patients in south of Xinjiang who were screened for early cervical cancer.Liquid based cytology(TCT)combined with cell DNA was adopted as quantitative analysis,any positive results for the cervical biopsy were carried out pathologic examination,and the results were used as the gold standard in order to explore the sensitivity and accuracy of the liquid based cytology,cell DNA times analysis and liquid based cytology cell DNA times.Results 96 patients showed positive cervical biopsy value,including 42 cases of CINI,25 cases of CINII,18 cases of CINIII and 11 cases of cervical cancer.The positive rates of liquid-based cytology,cell DNA ploidy analysis and liquid-based cytology combined with cell DNA ploidy analysis were 71.03%,76.19%and 92.86%,respectively.The combined detection showed the highest sensitivity.The difference was statistically significant(P<0.05).TCT and DNA ploidy analysis showed that the combined sensitivity of the two methods was 75/96=78.13%,87.5%and 93.75%,respectively.The combined detection showed the highest sensitivity,and the difference was statistically significant(P<0.05).Conclusion The combination of DNA ploidy quantitative analysis with TLC-based cytology has high sensitivity for early cervical cancer screening,and it has high clinical value for cervical cancer screening in the south of Xinjiang.
