单纯血尿IgA肾病病理与炎症因子关联性研究

摘　　要：目的通过探讨单纯血尿IgA肾病患者病理与炎症标志物间的关联性,以期寻找重复性、可操作性强的生物指标来充分评估病情。方法分析122例患者的肾脏病理指标、半定量积分Katafuchi积分与临床血清、尿液标本炎症相关指标(肾小管损伤指标,如N-乙酰-β-氨基葡萄糖苷酶(N-acetyl-β-D-glucosaminidase,NAG)、肾损伤分子-1(kidney injury molecule-1,KIM-1)和炎症反应因子,如H因子、单核细胞趋化蛋白-1(monocyte chemotactic protein,MCP-1)、细胞间粘附分子-1(intercellular adhesion Molecule,ICAM-1)和血管细胞粘附分子-1(vascular cell adhesion molecule,VCAM-1)的关联性。结果尿液H因子、VCAM-1和ICAM-1与肾小球活动性病病理指标正相关,其中ICAM-1显著相关。与小球活动性病变相关有H因子、VCAM-1和ICAM-1,而与KIM-1、NAG不相关。与小管活动性指标相关的是H因子和NAG,与血管病变相关的是KIM-1。血清炎症标志物与肾小球或肾小管损活动性伤无相关性。结论单纯血尿的IgA肾病患者尿炎性标志物可以作为评价单纯血尿肾小管急性损伤性病变的候选生物标志物。Objective To explore the relationship between pathology and inflammatory markers in patients with hematuria IgA nephropathy and to find reproducible and operable biomarkers.Methods Renal pathological indexes,semi-quantitative integral katafuchi scores and inflammation related indexes(renal tubular injury index)in clinical serum and urine were analyzed in 122 patients.Results H factors,vcam-1 and icam-1 in urine were positively correlated with glomerular active venereal disease,and icam-1 was significantly correlated.H factors,vcam-1 and icam-1 were associated with glomerular active lesions,but they were not associated with kim-1 nag.H factors and nags were correlated with tubule activity,and kim-1 was associated with vascular lesions.There was no correlation between serum inflammatory markers and glomerular or tubular injury.Conclusions The urinary inflammatory markers in IgA nephropathy patients with hematuria alone can be used as candidate biomarkers for the evaluation of acute renal tubular injury in patients with hematuria alone.
