护理专案改善降低鼻胃肠管非计划性拔管率效果的研究

摘　　要：目的探讨护理专案改善行动降低患者鼻胃肠管非计划性拔管率的效果。方法选取2015年1月至12月留置鼻胃肠管患者为对照组,2016年1月至2016年12月留置鼻胃肠管患者为干预组,采用护理专案改善的方法进行干预,从管路评估、固定妥善等方面进行效果比较,并随机访谈病区护士对预防管路滑脱护理专案改善的意见。结果实施护理专案改善后,干预组鼻胃肠管非计划性拔管发生率较对照组明显下降(P<0.01),预防管路滑脱关键监测指标落实率明显提升(P<0.01或P<0.05),护理专案改善提高了护士参与管理积极性。结论护理专案改善有利于针对非计划性拔管的高风险因素进行过程控制,实施有效干预措施,降低非计划性拔管率发生率,保障患者安全。Objective To explore the effect of nursing project in reducing the rate of unplanned extubation among patients with nasal gastrointestinal tract.Methods The patients with nasal gastrointestinal tube from January 2015 to December 2015 were selected as the control group,and the patients with the nasal gastrointestinal tube from January 2016 to December 2016 were selected as the intervention group.The intervention group used the nursing project to assess the rate of unplanned extubation.Then compared the pipeline evaluation and fixation between two groups,and interviewed nurses'opinions on nursing project.Results The rate of unplanned extubation in the intervention group was significantly lower than those of the control group(P<0.01),and the implement rate of key monitoring indexes for slippage prevention in the intervention group were significantly higher than those of the control group(P<0.01 or P<0.05).The nursing project improved the enthusiasm of nurses to participate in management.Conclusion The nursing project is beneficial to the progress control of high risk factors,implement effective measures,reduce the rate,and ensure the patients safety of unplanned extubationamong patients with nasal gastrointestinal tract.
