益气聪明汤治疗老年痴呆的Meta分析

摘　　要：目的:系统评价益气聪明汤治疗老年痴呆的疗效与安全性。方法:系统检索CochraneLibrary、PubMed、EMbase、维普数据库(VIP)、中国知网数据库(CNKI)、万方数据知识服务系统(WanFangData)自收录以来至2019年3月发表的益气聪明汤治疗老年痴呆的随机对照试验,并用Cochrane协作网提供的ReviewManager5.3软件对纳入的文献进行Meta分析。结果:最终纳入10个研究,共764例患者。对10个研究进行合并分析:益气聪明汤组总有效率与常规西药组比有优势[OR=3.37,95%CI=(2.35,4.83);Z=6.62,P<0.00001];MMSE、ADL、ADAS-cog量变评分与西药组相比,差异有统计学意义,分别为[WMD=0.48,95%CI=(0.31,0.64);Z=5.64,P<0.00001]、[WMD=-0.52,95%CI=(-0.76,-0.28);Z=4.31,P<0.0001]、[WMD=-0.41,95%CI=(-0.71,-0.11);Z=2.71,P=0.007]。结论:现有的证据表明益气聪明汤能提高老年痴呆的临床疗效,改善认知功能和日常生活自理能力,且不良反应较小。但纳入研究数量偏少且质量偏低,尚需要更多高质量文献以验证其疗效。Objective:To systematically assess the efficacy and safety of Yiqi Congming decoction in treating senile dementia.Methods:The database of Cochrane Library,Pubmed,EMbase,VIP,CNKI,WanFang were systematic searched for the randomized controlled trials about Yiqi Congming decoction in the treatment of senile dementia,published before March 2019.Meta-analysis of the incorporated literature were conducted by Review Manager 5.3 software from the Cochrane Collaboration.Results:10 RCTs included a total of 764 patients.The combined analysis of 10 studies:Yiqi Congming decoction group has an advantage in total efficiency compared with conventional western medicine group[OR=3.37,9 5%CI=(2.35,4.83);Z=6.62,P<0.00001];The MMSE,ADL,and ADAS-cog quantitative change scores were statistically significant compared with the western medicine group,respectively below[WMD=0.48,9 5%CI=(0.31,0.64);Z=5.64,P<0.00001],[WMD=-0.52,9 5%CI=(-0.76,-0.28);Z=4.31,P<0.0001],[WMD=-0.41,9 5%CI=(-0.71,-0.11);Z=2.71,P=0.007].Conclusion:The available evidence suggests that Yiqi Congming decoction can improve the clinical efficacy of senile dementia,improve cognitive function and s elf-care ability in daily life,and have fewer adverse reactions.However,the quantity and quality of included studies are low,more highquality literature are still needed to verify its efficacy.
