醒脑开窍针法联合丰富康复训练对缺血性脑卒中患者神经功能康复的作用

摘　　要：目的:评价醒脑开窍针法联合丰富康复训练治疗缺血性脑卒中患者的康复疗效。方法:将80例缺血性脑卒中患者随机分为丰富康复组(40例)和针刺联合丰富康复组(40例),2组患者均采用常规药物治疗和丰富康复训练,针刺联合丰富康复组在此基础上增加针刺治疗。治疗前和治疗后分别采用神经功能缺损程度、Fugl-Meyer评定量表(FMA)、蒙特利尔认知评估量表(Mo CA)和改良Barthel指数(MBI)进行评估。结果:4个疗程后,2组神经功能缺损评分均降低(P<0.01),FMA、Mo CA、MBI评分均提高(P<0.01)。针刺联合丰富康复组神经功能缺损评分低于丰富康复组(P<0.05),FMA、Mo CA、MBI评分均高于丰富康复组(P<0.05)。结论:醒脑开窍针法联合丰富康复训练和丰富康复训练均有助于促进缺血性脑卒中神经功能恢复,改善运动、认知功能障碍,提高日常生活活动能力,且醒脑开窍针法联合丰富康复训练对缺血性脑卒中功能障碍改善更为显著。Objective: To investigate the effects of Xingnao Kaiqiao acupuncture combined with enriched rehabilitation training on neurological functions of patients with ischemic stroke.Methods 80 cases of ischemic stroke were randomly divided into acupuncture combined with enriched rehabilitation group(40 cases) and enriched rehabilitation group(40 cases). Both groups received routine medication and enriched rehabilitation training. the acupuncture combined with enriched rehabilitation group received acupuncture in addition.they were assessed with the intervention in the degree of nervous functional defects, simplified Fugl-Meyer Assessment(FMA), Montreal Cognitive Assessment(Mo CA), Modified Barthel Index(MBI) before and after treatment. Results After 4 courses of treatment, The scores of the intervention in the degree of nervous functional defects decreased in both groups after treatment(P<0.01), and the scores of FMA, Mo CA and MBI increased in both groups after treatment(P<0.01). The scores of the intervention in the degree of nervous functional defects decreased more in the acupuncture combined with enriched rehabilitation group than in the enriched rehabilitation group(P<0.05), and the scores of FMA, Mo CA and MBI increased more in the acupuncture combined with enriched rehabilitation group than in the enriched rehabilitation group(P<0.05). Conclusion:Xingnao Kaiqiao acupuncture combined with enriched rehabilitation and enriched rehabilitation all can promote the recovery of nerve function, improve motor and cognitive dysfunction, and improve the ability of daily living in patientswith ischemic stroke, and the effect of Xingnao Kaiqiao acupuncture combined with enriched rehabilitation on the dysfunction improving is more remarkable on ischemic stroke.
