五倍子枯矾方加减外洗联合维A酸乳膏治疗多发性掌跖疣的疗效分析

摘　　要：目的:探讨自拟中药方(五倍子、枯矾、皂荚等)外洗联合维A酸乳膏治疗多发性掌跖疣的临床疗效。方法:选取2017年6月至2018年8月间于我院皮肤科门诊就诊的153例多发性掌跖疣患者作为研究对象,根据患者的入院序号随机分成3组,即中药外洗组、维A酸乳膏组、中药外洗与维A酸乳膏联合用药组(下简称联合用药组),每组51例。分别在治疗前、3个疗程结束时、随访3个月时记录分析各组的皮损评分,统计疗效结果。结果:三组在治疗结束时皮损评分[维A酸乳膏组(8.28±4.95)、中药外洗组(5.32±3.40)、联合用药组(2.96±3.183)]较治疗前均降低(P<0.05),且联合用药组皮损评分低于另外两组(P<0.05),而维A酸乳膏组皮损评分高于另外两组(P<0.05);随访3个月时,维A酸乳膏组的皮损评分(10.86±2.95)高于另外两组[联合用药组(4.01±2.69)、中药外洗组(6.03±3.02)](P<0.05)。三组总有效率分别是维A酸乳膏组44.00%、中药外洗组74.00%、联合用药组90.00%,三组总有效率差异均有统计学意义(P<0.05)。两两相比,维A酸乳膏组总有效率分别低于另外两组(P<0.05),中药外洗组总有效率低于联合用药组(P<0.05)。随访3个月时复发率(维A酸乳膏组42.86%、中药外洗组12.50%、联合用药组7.69%),联合用药组低于维A酸乳膏组(P<0.05)。结论:通过中药外洗方加减可改善掌跖疣的皮损情况,提高其疗效,而与维A酸乳膏联合则效果更好。Objective:To investigate the curative efficiency of self-made prescription(gallnut,dired alum,gleditsia sinensis,etc.)combined with tretinoin cream in treating multiple plantar wart.Methods:153 outpatients with multiple plantar wart in the department of dermatology of our hospital from June 2017 to Auguest 2018 were selected.According to the admission number,the patients were randomly divided into three groups,which were traditional Chinese medicine(TCM)group,tretinoin cream group and TCM combined with tretinoin cream group(hereinafter referred as the combining group),each with 51 patients.The conditions on skin lesion were evaluated at pre-treatment,the end of 3 course of treatment and 3 months after treatment respectively.Results:Compared to pre-treatment,the scores of skin lesion in the three groups were significantly improved after completing 3 course of treatment,and scores in TCM group,the tretinoin cream group and the combining group were(5.32±3.40),(8.28±4.95)and(2.96±3.183)points respectively(P<0.05).Compared with other two groups,the combining group presented the lowest scores and the tretinoin cream group showed the highest scores(P<0.05).The total effective rate of the three groups was 44.00%in the tretionin cream group,74.00%in the TCM group,and 90.00%in the combing group.The total effective rate of the three groups had statistic difference(P<0.05).The total effective rate of the tretionin cream group was lower than the other two groups(P<0.05),and the total effective rate of the TCM group was lower than the combining group(P<0.05).The recurrence rate of the combining group(7.69%)was lower compared to that of the tretinoin cream group(42.86%)(P<0.05).Conclusion:The self-made prescription with addition and subtraction method may effectively improve the skin lesion conditions and curative efficiency for patients with multiple plantar wart.It shows a better outcome while combining with tretinoin cream.
