摘　　要：目的探讨宫腔镜、腹腔镜及B超引导下疤痕妊娠患者病灶清除及修补术的效果及安全性。方法从2017年5月至2018年5月期间收治的瘢痕妊娠患者中选择90例为研究对象,按照随机分组的原则将患者均分为宫腔镜组、腹腔镜组和B超组,在常规治疗的基础上三组患者分别在宫腔镜、腹腔镜及B超引导下实施病灶清除及修补术,术后观察患者手术有效率、手术状况、术后恢复状况及并发症发生状况等指标。结果术后腹腔镜组、宫腔镜组和B超组有效率分别为96.67%、80%和70%,且差异有统计学意义(P<0.05);在手术时间、出血量、医疗支出、住院时间、月经恢复时间、病灶缩小时间、β-hCG恢复至正常水平时间、阴道流血时间等指标上均为B超组最优,且术后并发症少,各组间差异均有统计学意义(均P<0.05)。结论腹腔镜组治疗有效率较高,但安全性较低;而B超组有效率较低,但安全性较高。Objective To explore the effect and safety of hysteroscopy,laparoscopy and B-mode ultrasound-guided lesion clearance and repair in patients with scar pregnancy.Methods Ninety patients with scar pregnancy admitted from May 2017 to May 2018 were selected as the research objects.According to the principle of random grouping,the patients were divided into hysteroscopy group,laparoscopy group and B-mode ultrasound group.On the basis of routine treatment,the three groups of patients underwent lesion clearance and repair under the guidance of hysteroscopy,laparoscopy and B-mode ultrasound respectively.The operative efficiency and surgical status of the patients were observed after operation.Conditions,post-operative recovery and complications were assessed.Results The effective rates of laparoscopic group,hysteroscopy group and B-mode ultrasound group were 96.67%,80% and 70% respectively,and the difference was statistically significant(P<0.05).In terms of operation time,bleeding volume,medical expenditure,hospitalization time,menstrual recovery time,focus reduction time,time of return to normal level of beta-hCG,vaginal bleeding time and other indicators,B-mode ultrasound group was the best,and had fewer complications after operation.There were significant differences between the two groups(all P<0.05).Conclusion The laparoscopic group had a higher effective rate,but a lower safety,while the B ultrasound group had a lower effective rate,but a higher safety.
