Ⅲ型鼻窦炎鼻息肉临床分期探讨以及各期手术疗效分析

摘　　要：目的分析Ⅲ型鼻窦炎鼻息肉临床分期特征以及各期手术疗效。方法选取在本院接受治疗的Ⅲ型鼻窦炎鼻息肉患者180例作为观察对象,对患者临床资料进行回顾性分析,结合患者的病史与病变程度等分为3期,对不同分期手术疗效进行分析。结果1期62例患者治疗总有效率为93.75%,2期76例患者治疗总有效率为81.58%,3期40例患者治疗总有效率为70.00%,1期治疗中有效率最高,其次为2期患者;1期患者仅有2例患者发生纸板损伤,2期患者有10例患者分别发生纸板损伤与鼻中隔脓肿等,3期患者有12例患者分别发生纸板损伤与鼻中隔脓肿等,三组患者并发症发生率之间存在显著差异。结论不同临床分期的Ⅲ型鼻窦炎鼻息肉,在解剖学改变及病变程度上均会存在一定差异,对于手术治疗难度及手术风险等具有一定影响,而3型患者的手术难度及风险均较高,且在术后比较容易发生并发症。Objective To analyze the clinical stages of type Ⅲ sinusitis and nasal polyps and the effect of different stages of operation.Methods 180 patients with type Ⅲ sinusitis and nasal polyps who were treated in our hospital were selected as the observation objects.The clinical data of the patients were retrospectively analyzed.Combined with the patient's history and the degree of pathological changes,the curative effect of different stages of surgery was analyzed.Results The total effective rate of 62 patients in stage 1 was 93.75%,76 patients in stage 2 were 81.58%,40 patients in stage 3 were 70.00%,the highest effective rate in stage 1 was 2 patients,only 2 patients in stage 1 had cardboard injury,10 patients in stage 2 had cardboard injury and nasal septum abscess respectively,and 12 patients in stage 3 had cardboard injury and nasal septum abscess respectively.There were significant differences in the incidence of complications between the three groups,such as cardboard injury and nasal septum abscess.Conclusion Different clinical stages of type Ⅲ sinusitis and nasal polyps have some differences in anatomical changes and pathological changes,which have certain effects on the difficulty of surgical treatment and surgical risk,while type Ⅲ patients have higher surgical difficulty and risk,and are more prone to complications after surgery.
