宫腔镜下子宫黏膜下肌瘤切除术后IVF-ET结局的病例分析

摘　　要：目的分析宫腔镜下子宫黏膜下肌瘤切除术后IVF-ET结局。方法该次研究中的观察对象均为方便选取该院接受治疗的子宫黏膜下肌瘤切除术患者,82例患者的收治时间均在2014年4月—2015年5月期间。依据上述患者所接受的手术治疗方案将其分为两组,对照组为开腹手术治疗,观察组患者接受宫腔镜下子宫黏膜下肌瘤切除术治疗。比较两组患者术后体外受精-胚胎移植(IVF-ET)结局。结果观察组患者的活产占比(82.93%)明显较对照组(29.27%)高(χ^2=23.970,P=0.010);观察组患者的生化妊娠、未妊娠以及异位妊娠占比分别为0.00%、0.00%以及4.88%,均较对照组(9.76%、17.07%以及21.95%)低(P=0.040、0.010、0.020<0.05);两组患者的早产、早期流产、晚期流产占比比较,差异有统计学意义(P>0.05)。结论宫腔镜下子宫黏膜下肌瘤切除术属于临床治疗子宫黏膜下肌瘤的金标准方式,且该术式可以将患者术后IVF-ET结局改善。Objective To analyze the outcome of IVF-ET after hysteroscopic uterine submucous myoma resection.Methods The subjects in this study were all patients with uterine submucosal myoma resection treated in the hospital were convenient selected.The admission time of 82 patients was from April 2014 to May 2015.According to the surgical treatment plan accepted by the above patients,the patients were divided into two groups.The control group was treated with open surgery.The patients in the observation group underwent hysteroscopic submucosal myomectomy.The in vitro fertilization-embryo transfer(IVF-ET)outcomes were compared between the two groups.Results The proportion of live births in the observation group(82.93%)was significantly higher than that in the control group(29.27%)(χ^2=23.970,P=0.010).The biochemical pregnancy,non-pregnancy and ectopic pregnancy ratio of the observation group 0.00%,0.00%,and 4.88%,respectively,were lower than the control group(9.76%,17.07%,and 21.95%)(P=0.040,0.010,0.020<0.05);premature delivery,early abortion,and late abortion ratio in both groups was compared,the difference was statistically significant(P>0.05).Conclusion Hysteroscopic submucosal myomectomy is a gold standard method for clinical treatment of uterine submucosal fibroids,and this procedure can improve the outcome of IVF-ET after surgery.
