锚钉内固定治疗后交叉韧带胫骨止点撕脱性骨折的临床效果探究

摘　　要：目的研究锚钉内固定治疗后交叉韧带胫骨止点撕脱性骨折的临床效果。方法方便选择2014年3月—2017年9月期间于该院骨科接受治疗的交叉韧带胫骨止点撕脱性骨折患者50例,入组患者经膝关节内侧倒“L”形入路切开复位,使用锚钉内固定治疗,术后使用石膏托固定膝关节屈曲30°(4周),将石膏拆除外固定适时锻炼膝关节屈膝功能。结果50例患者中,完成随访共48例,患者于术后2~4个月骨性,平均(3.02±1.01)个月;术前评分为(45.32±3.45)分,术后为(89.67±5.67)分,手术前后膝关节评分对比差异有统计学意义(t=46.991,P=0.000);其中优30例、良15例、可3例,优良率为93.75%(45/48)。结论行膝后内侧“L”形入路经锚钉内固定方法治疗交叉韧带胫骨止点撕脱性骨折患者疗效相当,且内固定可靠、安全性高、稳定膝关节,有利于促使膝关节功能的恢复。Objective To study the clinical effect of internal fixation with anchor for the treatment of posterior cruciate ligament avulsion fracture of the humerus.Methods Convenient selected a total of 50 patients with avulsion fractures of the cruciate ligament and humeral fractures who underwent orthopedics in the hospital from March 2014 to September 2017 were enrolled.The patients underwent open reduction through the L-shaped approach of the knee joint.The anchor was used for internal fixation,and the knee joint was flexed for 30 degree(4 weeks)after the operation.The gypsum was removed and externally fixed to exercise the knee flexion function.Results Of the 50 patients,48 patients were followed up.The patients were bony at 2 to 4 months after surgery,with an average of(3.02±1.01)months.The preoperative score was(45.32±3.45)points and the postoperative rate was(89.67±5.67)points.There were statistically significant differences in knee joint scores before and after surgery(t=46.991,P=0.000).Among them,30 cases were excellent,15 cases good,and 3 cases fair.The excellent and good rate was 93.75%(45/48).Conclusion The posterior medial"L"shaped approach for the treatment of cruciate ligaments with avulsion fractures of the cruciate ligaments is effective,and the internal fixation is reliable,the safety is high,the knee joint is stabilized,and the knee joint function recovery is promoted.
