超声对乳腺癌患者术后局部复发监测的应用研究

摘　　要：目的评价超声对乳腺癌手术史患者术后监测、评估局部复发的临床价值。方法选取自2013年1月-2016年12月间在本院接受乳腺癌手术的157例患者,其中115例患者接受乳房切除术,42例患者接受乳腺保乳术。截至2016年12月共回顾了1645次超声检查。阳性病变的确诊是基于细胞病理学结果。比较检查为阳性和未检查为阳性的患者向远处转移的发生率差异。结果在115例乳房切除术患者的1179次检查中,27名患者最终诊断为超声阳性34次,最终阳性9次(33.3%)。在42名保乳手术患者的472次检查中,9名患者超声检查有13次阳性,最终阳性1次(11.1%)。在乳房切除术患者中,最终阳性发现的患者比没有最终阳性发现的患者有更高的远处转移发生率(9例患者中的3例,33.3%,106例患者中的10例,9.4%,p=0.011)。保乳手术患者无远处转移。结论术后超声对乳腺及其区域恶性病变的检测具有较高的敏感性,可以作为乳房切除术患者远处转移的预测指标,但对保乳手术患者来说,术后超声在恶性病变检测中的作用尚不清楚。Objective To evaluate the clinical value of ultrasound on postoperative monitoring of patients undergoing breast cancer surgery and to evaluate the local recurrence.Methods 157 patients who underwent breast cancer surgery in our hospital from January 2013 to December 2016 were chosen.Among them,115 patients underwent mastectomy and 42 patients underwent breast conserving surgery.As of December 2016,a total of 1645 ultrasound examinations had been reviewed.The diagnosis of positive lesions was based on the results of cytopathology.The incidence of distant metastasis was compared between those who were positive and those who were not positive.Results Among 1179 examinations in 115 patients undergoing mastectomy,27 patients were diagnosed as positive for ultrasound 34 times and finally 9 times(33.3%).Among the 472 examinations in 42 breasts conserving patients,13 were positive,finally 1(11.1%)was positive by ultrasound.In patients with mastectomy,the incidence of distant metastasis was higher in patients with final positive findings than in patients without final positive findings(3 out of 9 patients,33.3%,10 out of 106 patients,9.4%,P=0.011).There was no distant metastasis in breast conserving patients.Conclusion Postoperative ultrasound has a high sensitivity to the detection ofmalignant lesions of breast and its region,and can be used as a predictive index of distant metastasis in patients undergoing mastectomy,but the role of postoperative ultrasound in the detection ofmalignant lesions is not clear for patients undergoing breast conserving surgery.
