摘 要:目的分析评价西藏自治区碘缺乏病20年(1997-2016年)综合防治效果,研究探讨巩固和完善碘缺乏病防控长效机制。方法按照各年《全国碘缺乏病病情监测项目实施方案》的要求,通过统计各年碘盐覆盖率和合格碘盐食用率、8~10岁儿童和孕妇尿碘中位数、儿童甲状腺肿大率三大项指标进行碘缺乏病综合防治效果分析与评价。结果1997-2016年碘盐覆盖率和合格碘盐食用率分别从19.0%、8.8%提升至97.4%、91.3%;儿童和孕妇尿碘水平分别由55.4μg/L、27.0μg/L提升至162.3μg/L、129.1μg/L;儿童甲状腺肿大率从29.0%下降至0.4%。结论20年来所采取的综合防治措施是实现可持续消除碘缺乏病较为有效的方法,但同时应持续加强碘盐生产与流通、配送环节的严格管控,提升偏远地区碘缺乏病健康知识知晓率,切实有效提升和巩固合格碘盐食用率以及孕产妇碘营养水平。Objective To analyze and evaluate the comprehensive prevention and treatment effect of iodine deficiency disease in Tibet Autonomous Region in the past 20 years(1997-2016),and to study and explore the long-term mechanism for strengthening and improving the prevention and control of iodine deficiency disease in Tibet.Methods According to the requirements of the annual National Implementation Plan for Iodine Deficiency Disease Condition Monitoring Projects(IDD),a survey of comprehensive prevention effect and treatment of iodine deficiency disorders was conducted by 3 statistical indicator:the iodized salt coverage rate and qualified iodized salt consumption rate,the median urinary iodine of children aged 8~10 years and pregnant women and the goiter rate of children.Results From 1997 to 2016,the consumption rate of iodized salt coverage and the rate of qualified iodized salt have increased from 19.0%to 97.4%and 8.8%to 91.3%respectively.The urine and iodine levels in children and pregnant women have increased from 55.4μg/L to 162.3μg/L and 27.0μg/L to 129.1μg/L respectively.The rate of thyroid swelling in children has decreased from 29.0%to 0.4%.Conclusion The comprehensive prevention and control measures which had adopted in the past 20 years are effective ways to achieve sustainable elimination of IDD.What’s more,we should continue to strengthen strict control over production,circulation and distribution of iodized salt,improve the awareness of health knowledge of iodine deficiency disorders in remote areas to effectively improve and consolidate the consumption rate of qualified iodized salt and the maternal iodine nutrition level of pregnant women.
