《神应经》腰痛三穴温针灸联合腰俞隔附子饼灸治疗气虚血瘀型慢性腰肌劳损临床观察

摘　　要：目的:观察《神应经》腰痛三穴温针灸联合腰俞隔附子饼灸治疗气虚血瘀型腰肌劳损的临床疗效及对血液黏度指标、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)的影响。方法:采用随机方法将110例患者分为对照组和观察组,各55例。对照组口服双氯芬酸钠双释放肠溶胶囊和双氯芬酸二乙胺乳胶剂外用;观察组给予《神应经》腰痛三穴温针灸联合腰俞隔药灸治疗。两组疗程均为4周。治疗前后两组进行中医症状评分和腰部功能状态评分,并统计临床疗效,检测血液黏度指标、TNF-α、IL-6。结果:观察组临床疗效总有效率为92.73%,明显高于对照组(74.55%),差异比较有统计学意义(P<0.05);观察组治疗后中医症状评分、腰部功能状态评分、血液黏度指标、血清TNF-α、IL-6水平明显低于对照组,差异比较均有统计学意义(均P<0.05)。结论:《神应经》腰痛三穴温针灸联合腰俞隔附子饼灸能缓解气虚血瘀型腰肌劳损患者的腰痛,改善腰部功能,改善微循环障碍,减轻炎症反应,临床疗效显著。Objective:To observe the clinical effect of needle-warming moxibustion combined with Fuzi-padded moxibustion in treating chronic lumbar muscle strain(LMS)of qi deficiency and blood stasis,and its influence to blood viscosity indexes,tumor necrosis factor-alpha(TNF-α)and interleukin-6(IL-6).Methods:110 patients who met the inclusion criteria were randomly divided into the control group(n=55)and the observation group(n=55).The control group was treated with diclofenac sodium double-release enteric-coated capsules and diclofenac diethylamine latex for external use.The observation group was treated with needle-warming moxibustion on three lumbar pain points,combined with Fuzi-padded moxibustion on points of Yaoshu,which recorded in Shenying Jing.Both groups were treated for four weeks.TCM symptom scores and lumbar function scores were assessed,the serum levels of TNF-α,IL-6 and boood viscosity were measured before and after the treatment,and the clinical efficacy was evaluated in the two groups.Results:The total effective rate of the observation group was 92.73%,which was significantly higher than 74.55%of the control group(P<0.05).TCM symptom scores,lumbar function scores,blood viscosity indexes and serum levels of TNF-αand IL-6 were significantly lower in the observation group than those in the control group(P<0.05).Conclusion:Needle-warming moxibustion combined with Fuzi-padded moxibustion,which was recorded in Shenying Jing,can alleviate lumbago,improve lumbar function and microcirculation,reduce inflammatory reaction in patients with chronic LMS of qi deficiency and blood stasis.The clinical effect is remarkable.
