摘　　要：目的综合评价度洛西汀联合脉冲射频(pulse radiation frequency,PRF)治疗带状疱疹后神经痛(postherpetic neuralgia,PHN)的效果。方法将入组的103例PHN患者随机分为两组,试验组和对照组。其中试验组52例,行度洛西汀加PRF联合治疗;对照组51例,行PRF治疗。随访术前和术后1周,1、3、12、24个月,记录两组患者的疼痛情况(VAS)、抑郁情况(SDS)和失眠情况(PSQI),并进行统计学分析。结果试验组在术后1周,1、3、12、24个月的VAS、SDS和PSQI指标均较术前明显改善(P<0.05);对照组在术后1周,1、3、12、24个月的VAS评分均较术前明显改善(P<0.05),但对照组SDS和PSQI术后1周较术前无显著改善(P>0.05),在术后1、3、12、24个月较术前明显改善(P<0.05)。组间比较,试验组VAS、SDS和PSQI改善情况均优于对照组(P<0.05)。结论度洛西汀联合PRF治疗PHN在改善患者疼痛、抑郁和失眠方面效果显著,近远期疗效均优于PRF治疗。Objective To evaluate the efficacy of duloxetine combined with pulse radiation frequency(PRF)in the treatment of postherpetic neuralgia(PHN).Methods One hundred and three patients with PHN were randomly divided into two groups:experimental group and control group.There were 52 patients in the experimental group who received duloxetine combined with PRF,and 51 patients in the control group were treated with PRF treatment.All patients were followed up for preoperative and 1 week,1 month,3 months,12 months and 24 months after operation.VAS,SDS and PSQI were recorded and analyzed statistically.Results VAS,SDS and PSQI were significantly improved in 1 week,1 month,3 months,12 months and 24 months after operation in experimental groups(P<0.05),the VAS scores of the control group were also significantly improved compared with those before operation(P<0.05).In the control group,there was no significant improvement in SDS and PSQI 1 week after operation(P>0.05),however,it was significantly improved 1,3,12 and 24 months after operation(P<0.05).The improvement of VAS,SDS and PSQI in the experimental group was significantly better than that in the control group(P<0.05).Conclusion It is effective that duloxetine combined with PRF in improving pain,depression and insomnia in patients with PHN,and its short-term and long-term effects are better than PRF.
