盐酸度洛西汀对玫瑰痤疮疗效及焦虑抑郁的影响

摘　　要：目的探讨玫瑰痤疮患者口服盐酸度洛西汀片治疗的临床效果,分析焦虑抑郁情绪与疾病的相关性。方法将符合临床诊断的128例患者随机分为两组。对照组64例,采用口服盐酸多西环素、外用甲硝唑凝胶治疗;观察组64例,在对照组用药的基础上加用盐酸度洛西汀口服。结果患者的病情程度与焦虑、抑郁程度呈正相关性(P<0.05);观察组在治疗后4、8、12周的有效率(50.00%、81.25%、92.19%)和痊愈率(12.50%、35.94%、57.81%)均明显高于同期对照组的有效率(28.13%、50.00%、71.88%)和痊愈率(3.13%、7.81%、35.94%),各阶段疗效均得到显著提升(P<0.05或P<0.01);观察组治疗后中度和重度玫瑰痤疮患者的痊愈率和总显效率明显高于对照组(P<0.05),且重度患者的总显效率提升更显著(P<0.01);观察组HAMA、HAMD 24项、DLQI的评分分值改善程度较对照组更明显(P<0.01);观察组累计复发率(7.02%)明显低于对照组(23.91%),远期疗效稳定(P<0.05);治疗中两组患者所出现的不良反应轻微,差异无统计学意义(P>0.05)。结论盐酸度洛西汀治疗玫瑰痤疮综合疗效显著,安全可靠,且对中重度患者的治疗更具临床价值,通过改善患者焦虑抑郁情绪,有效降低了复发率,促进了康复。Objective To investigate the clinical effects of oral duloxetine hydrochloride administered in patients with rosacea,and analyze the correlation between anxiety/depression and rosasea.Methods One hundred and twenty eight patients were divided into two groups randomly.In the control group,64 patients were treated with oral doxycycline hydrochloric acid and topical Metronidazole Gel.The observation group were given oral duloxetine hydrochloride on the basis of the treatment protocols.Results It has a positive correlation between the severity of rosacea and anxiety/depression(P<0.05).Total excellence rate(50.00%,81.25%,92.19%)and the recovery rate(12.50%,35.94%,57.81%)of the patients in observation group were significantly higher than those in control group(28.13%,50.00%,71.88%)and the recovery rate(3.13%,7.81%,35.94%)in the same period,after 4,8,12 weeks treatment.The scores of HAMA,HAMD 24 and DLQI in the observation group improved more significantly than those in the control group(P<0.05);the cumulative recurrence rate in the observation group(7.02%)was significantly lower than that in the control group(23.91%)and the long-term effect was better(P<0.05);the mild adverse reactions were found in the two groups,and the difference was not statistically significant(P>0.05).Conclusion Duloxetine hydrochloride is effective and safe in the treatment of rosacea,and it is more clinically valuable for the treatment of moderate to severe patients.By improving the anxiety and depression of patients,the recurrence rate is effectively reduced and rehabilitation is promoted.
