宣白承气汤治疗慢性阻塞性肺疾病急性加重痰热壅肺证临床疗效及对C反应蛋白、降钙素原和中性粒细胞百分比的影响

摘　　要：目的观察宣白承气汤治疗慢性阻塞性肺疾病急性加重(AECOPD)痰热壅肺证临床疗效及对C反应蛋白(CRP)、降钙素原(PCT)和中性粒细胞百分比的影响。方法将100例AECOPD痰热壅肺证患者按照随机数字表法分为2组。对照组50例予常规西医治疗;治疗组50例在对照组治疗基础上联合宣白承气汤治疗。2组疗程均为10 d。比较2组治疗前后CRP、PCT水平及中性粒细胞百分比,并统计疗效。结果治疗组总有效率96.00%,对照组总有效率80.00%,治疗组疗效优于对照组(P<0.05)。治疗组治疗后咳嗽、咯痰、喘息、胸闷、口干口渴、大便干结评分及主症积分、次症积分、总分均降低(P<0.01),对照组治疗后咳嗽、咯痰、喘息、胸闷、口干口渴评分及主症积分、次症积分、总分均降低(P<0.01,P<0.05)。治疗组治疗后咳嗽、咯痰、喘息、口干口渴、大便干结评分及主症积分、次症积分、总分均低于对照组(P<0.01,P<0.05)。治疗后2组CRP、PCT、中性粒细胞百分比均降低(P<0.05),治疗组治疗后均低于对照组(P<0.05)。结论宣白承气汤治疗AECOPD痰热壅肺证患者的疗效满意,可明显改善症状,降低CRP、PCT及中性粒细胞水平,有效抑制机体急性炎性反应,进而有助于患者康复。Objective To observe the clinical efficacy of Xuanbai Chengqi decoction on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)of phlegm-heat obstructing lung syndrome and the influence on C-reactive protein(CRP),procalcitonin(PCT)and neutrophils percentage.Methods 100 cases of AECOPD were divided into two groups according to the random number table method.50 cases in control group were treated by conventional western medicine.50 cases in treatment group were treated by Xuanbai Chengqi decoction on the basis of the control group.The course of treatment in both groups was 10 days.The CRP,PCT and neutrophils percentage before and after treatment were compared between the two groups,and the clinical efficacy of the two groups was counted.Results The total effective rate was 96.00%in treatment group and 80.00%in control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the scores of cough,expectoration,wheezing,chest distress,dry mouth and thirst,dry stool,main symptom score,secondary symptom score and total score in the treatment group were all decreased(P<0.01).After treatment,the scores of cough,expectoration,wheezing,chest distress,dry mouth and thirst,main symptom score,secondary symptom score and total scores in the control group were all decreased(P<0.01,P<0.05).After treatment,the scores of cough,expectoration,wheezing,dry mouth and thirst,dry stool,main symptom,secondary symptom and total scores in the treatment group were lower than those in the control group(P<0.01,P<0.05).After treatment,CRP,PCT and neutrophils in the two groups were all decreased(P<0.05),and those in the treatment group were lower than those in the control group(P<0.05).Conclusion Xuanbai Chengqi decoction has a satisfactory therapeutic effect in the treatment of AECOPD with phlegm-heat obstructing lung syndrome.It can obviously improve the symptoms,reduce the levels of CRP,PCT and neutrophils,effectively inhibit the acute inflammatory reaction of th
