基于体质分型洪氏外洗剂加减热敷对膝骨关节炎患者炎症因子和WOMAC骨关节炎指数评分的影响

摘　　要：目的观察不同体质分型洪氏外洗剂加减热敷对膝骨关节炎(KOA)患者炎症因子和西大略湖麦克马斯特大学(WOMAC)骨关节炎指数评分的影响。方法将120例KOA患者按照随机数字表法分为2组。治疗组60例根据体质分型阳虚质、痰湿质、血瘀质应用洪氏外洗剂加减热敷治疗;对照组60例不同体质分型均予洪氏外洗剂热敷治疗。2组均治疗2周。比较2组疗效;观察2组治疗前后炎症因子白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)水平及WOMAC骨关节炎指数评分变化。结果治疗组临床控制率68. 3%,对照组临床控制率48. 3%,治疗组疗效优于对照组(P <0. 05)。2组阳虚质、痰湿质、血瘀质治疗后IL-1β、TNF-α水平及WOMAC骨关节炎指数评分均较本组治疗前降低(P <0. 05),且治疗组降低更明显(P <0. 05)。结论不同体质分型洪氏外洗剂加减热敷治疗KOA,能改善患者临床症状,降低WOMAC骨关节炎指数评分,降低IL-1β、TNF-α水平,提高疗效。Objective To observe the effect of Hongshi external lotion and hot compress based on constitution classification on the inflammatory factors and west lake luck McMaster university(WOMAC)osteoarthritis index scores in patients with knee osteoarthritis(KOA).Methods 120 patients with KOA were randomly divided into two groups according to the random number table method.60 cases in treatment group were treated by Hongshi external lotion and hot compress according to the constitution type of yang deficiency,phlegm dampness and blood stasis.60 cases in control group were treated by Hongshi external lotion and hot compress according to the constitution type.Both groups were treated for 2 weeks.The level of inflammatory factor interleukin1β(IL-1β),tumor necrosis factorα(TNF-α)and the scores of WOMAC osteoarthritis index were observed before and after treatment between the two groups.Results The clinical control rate was 68.3%in the treatment group and 48.3%in the control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).The levels of IL-1β,TNF-αand the scores of WOMAC osteoarthritis index of yang deficiency,phlegm dampness and blood stasis constitution in the two groups were lower than those in the group before treatment(P<0.05),and the decrease was more significant in the treatment group(P<0.05).Conclusion Different types of Hongshi external lotion and hot compress can improve the clinical symptoms,reduce the WOMAC osteoarthritis index,reduce the levels of IL-1βand TNF-α,and improve the efficacy of KOA.
