额肌瓣悬吊术与上睑提肌缩短术矫治上睑下垂疗效比较

摘　　要：目的:探究额肌瓣悬吊术对于上睑下垂患者的治疗效果和术后并发症的影响。方法:选取2017年5月-2019年5月笔者医院眼科收治的60例上睑下垂患者,随机分为对照组(30例)和观察组(30例)。对照组行上睑提肌缩短术,观察组行额肌瓣悬吊术,比较两组疗效、上睑回退量、眼睑闭合不全及瞬目次数、手术并发症发生率。结果:观察组总有效率为93.33%,高于对照组的66.67%,差异有统计学意义(χ2=5.100,P=0.020)。观察组术后6个月、术后9个月、术后12个月的上睑回退量均显著低于对照组,差异均有统计学意义(P<0.05)。术后1d和术后10d的观察组眼睑闭合不全量均显著小于对照组,瞬目次数均显著多于对照组,差异均有统计学意义(P<0.05)。观察组并发症总发生率为6.67%,低于对照组的30.00%,差异有统计学意义(χ2=4.010,P=0.049)。结论:与上睑提肌缩短术比较,额肌瓣悬吊术对于上睑下垂患者具有较好的矫治效果,可有效改善上睑下垂患者眼睑闭合不全情况,避免上睑回退,可降低术后并发症发生率。Objective To explore the therapeutic effect of frontalis muscle flap suspension on blepharoptosis patients and the application value of postoperative complications.Methods From May 2017 to May 2019,60 patients with blepharoptosis were randomly divided into the control group(30 cases)and the observation group(30 cases).The control group was treated with levator palpebrae superioris muscle shortening operation,while the observation group was treated with frontalis muscle flap suspension operation.The curative effect,upper eyelid retraction amount,eyelid closure insufficiency,blink frequency and incidence rate of surgical complications were compared between the two groups.Results The total effective rate of the observation group was 93.33%higher than that of the control group(66.67%),the difference was statistically significant(χ2=5.100,P=0.020).The amount of upper eyelid retraction in the observation group was significantly lower than that in the control group at 6 months,9 months and 12 months after operation(P<0.05).The number of blinks in the observation group was significantly higher than that in the control group(P<0.05).The total incidence of complications in the observation group was 6.67%,lower than that in the control group(30.00%,χ2=4.010,P=0.049).Conclusion Comparing with levator palpebrae superioris muscle shortening surgery,frontalis muscle flap suspension surgery has better correction effect in the treatment of severe blepharoptosis,and can effectively improve the eyelid closure insufficiency and blink condition of patients.
