黄芪桂枝五物汤加味辅治糖尿病肾病早期临床观察

摘　　要：目的:观察黄芪桂枝五物汤加味辅助治疗早期糖尿病肾病的临床疗效。方法:将早期糖尿病肾病患者60例随机分为治疗组和对照组各30例,2组患者均进行基础治疗,包括健康教育,低盐低脂饮食,同时口服降糖药物或皮下注射胰岛素治疗,并根据3餐前血糖水平进行调整。治疗组患者在此基础上加服黄芪桂枝五物汤。观察2组患者疗效及空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)水平,血肌酐、尿微量白蛋白、尿蛋白水平,总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)以及高密度脂蛋白(HDL)水平。结果:总有效率观察组为86.7%,对照组为63.3%,2组比较差异有统计学意义(P<0.05)。2组治疗后FPG、2 h PG、Hb A1c水平均较治疗前下降(P<0.05),治疗组低于对照组(P<0.05)。2组患者治疗后血肌酐、尿微量白蛋白、尿蛋白水平均下降(P<0.05),治疗组下降幅度大于对照组(P<0.05)。治疗后除HDL外,2组TC、TG、LDL下降(P<0.05),治疗组较对照组下降更明显(P<0.05)。结论:黄芪桂枝五物汤加味辅助治疗能够改善早期糖尿病肾病患者临床症状及肾功能,效果优于基础治疗。Objective: To observe clinical effects of modified Huangqi Guizhi Wuwu decoction as adjunctive therapy in treating early diabetic nephropathy(DNP). Methods: Sixty patients were randomized into the treatment group and the control group, 30 cases each group, both groups accepted basic therapy including health education,low-salt low-fat diet, oral administration of hypoglycemic drugs or hypodermic injection of insulin simultaneously,adjusting according to the levels of blood glucose before three meals. The treatment group took Huangqi Guizhi Wuwu decoction based on the therapy given to the control group. To observe clinical effects, the levels of FPG, 2 h PG,HbA1 c, serum creatinine(Scr), microalbuminuria(MAU), urine protein, TC, TG, LDL and HDL in both groups.Results: Total effective rate of the observation group was 86.7%, higher than 63.3% of the control group, and the difference had statistical meaning(P<0.05). The levels of FPG, 2 h PG and HaA1 c of both groups after treating reduced compared with before treating(P<0.05), the treatment group was lower than the control group(P<0.05).The levels of Scr, MAU and urine protein reduced in both groups after treating(P <0.05), the decrease of the treatment group was larger than that of the control group(P<0.05). After treating, except HDL, TC, TG and LDL in both groups lowered(P<0.05), the decrease of the treatment group was more notable compared with the control group(P <0.05). Conclusion: Modified Huangqi Guizhi Wuwu decoction as adjunctive therapy could improve clinical symptoms and renal function of early DNP, and its effects are superior to these of basic therapy.
