摘　　要：目的:观察应用导向器辅助下带袢钢板重建喙锁韧带联合修复肩锁韧带克氏针固定及中医辨证分期治疗RockwoodⅢ型肩锁关节脱位的临床疗效。方法:采用等距随机抽样法将Rockwood分型Ⅲ型肩锁关节脱位患者36例分为接受带袢钢板重建喙锁韧带联合修复肩锁韧带克氏针固定治疗的对照组和在此基础上进行中医分期治疗的观察组各18例,观察2组临床疗效及VAS评分、FMA评分、肩关节活动度情况。结果:优良率观察组为94.4%,对照组为83.3%,2组比较差异有统计学意义(P<0.05);治疗后2组患者VAS评分、FMA评分及肩关节活动度比较差异有统计学意义(P<0.05)。结论:带袢钢板重建喙锁韧带联合修复肩锁韧带克氏针固定及中医辨证分期治疗Rockwood分型Ⅲ型肩锁关节脱位疗效优于带袢钢板重建喙锁韧带联合修复肩锁韧带克氏针固定治疗。Objective: To explore clinical effects of coracoclavicular ligament reconstruction using looped steel plate, coracoclavicular ligament repair, fixation using Kirschner wire combined with TCM phased therapy in the treatment for Rockwood Ⅲ type acromioclavicular joint dislocation assisted by the guide. Methods: Using isometric random sampling method, 36 patients were divided into the control group, who accepted coracoclavicular ligament reconstruction using looped steel plate, coracoclavicular ligament repair, fixation using Kirschner wire, and the observation group, who received TCM phased therapy, 18 cases each group, to observe clinical effects, VAS scores, FMA scores and shoulder joint activity between both groups. Results: Excellent rate of the observation group was 94.4%, higher than 83.3% of the control group, and the difference had statistical meaning(P<0.05);the difference could be found in VAS scores, FMA and shoulder joint activity between both groups after treating(P<0.05).Conclusion: Coracoclavicular ligament reconstruction using looped steel plate, coracoclavicular ligament repair, fixation using Kirschner wire combined with TCM phased therapy in the treatment for Rockwood Ⅲ type acromioclavicular joint dislocation are superior to coracoclavicular ligament reconstruction using looped steel plate, coracoclavicular ligament repair and fixation using Kirschner wire.
