加味泽泻汤配合Epley复位法治疗后半规管良性位置性眩晕临床观察

摘　　要：目的:观察加味泽泻汤配合Epley复位法治疗后半规管良性位置性眩晕的临床疗效。方法:将128例确诊的后半规管良性位置性眩晕患者采用随机数字表法分为两组各64例,治疗组给予加味泽泻汤+盐酸倍他司汀+Epley复位法治疗;对照组给予盐酸倍他司汀+Epley复位法治疗。10天为1个疗程,并随访6个月,观察两组治疗前后、随访6个月不同时间点PSC-BPPV疗效、复发率、中医证候疗效、生存质量评分。结果:两组治疗后、随访6个月中医证候评分、生存质量比较均较治疗前减低(P﹤0.05);且治疗组在眩晕昏蒙、视物旋转、胸脘痞闷、恶心呕吐、肢体困重评分、机体功能、生活情感、躯体活动度评分等方面均低于对照组(P﹤0.05);治疗组有效率为93.7%,对照组为88.3%;两组比较差异有统计学意义(Z=-2.058,P=0.040)。结论:加味泽泻汤配合Epley复位能够明显减轻PC-BPPV患者临床症状,提高生存质量。Objectives:To observe the clinical effect of modified Zexie decoction and Epley reset method on posterior semicircular canal-induced benign positional vertigo(PSC-BPV).Methods:128 cases of diagnosed patients with PSC-BPV were randomly divided into two groups,64 cases in each group.The treatment group was treated with modified Zexie decoction and betahistine hydrochloride as well as Epley reset method,while the control group was treated with betahistine hydrochloride and Epley reset method.10 days as 1 course of treatment,and following up for 6 months.The scores of PSC-BPPV effect,recurrence rate,TCM syndrome and quality of life were observed in two groups before and after treatment as well as at different time points of following-up for 6 months.Results:After treatment and in following up for 6 months,the scores of TCM syndrome and quality of life in two groups were lower than before treatment(P﹤0.05);and in treatment group,the scores of dizziness,visual rotation,chest distress,nausea and vomiting,sleepy heavy limbs,body function,life emotion and body activity were lower than in control group(P﹤0.05);the effective rate was 93.7%in treatment group and 88.3%in control group respectively,and the comparative difference between two groups was statistically significant(Z=-2.058,P=0.040).Conclusions:Modified Zexie decoction combined with Epley reset method can significantly reduce the clinical symptoms of patients with PSC-BPV,improve the quality of life.
