摘 要:目的:检测miR-223和miR-155在类风湿关节炎(RA)患者血清外泌体中表达,探究其临床意义。方法:纳入2015年4月~2017年8月在本院就诊的RA患者77例为研究对象,选取同期在本院进行体检的健康者75例为对照。实时荧光定量(qRT)-PCR法检测血清外泌体中miR-223、miR-155水平,酶联免疫吸附试剂盒检测血清肿瘤免疫因子(TNF)-α、IL-6、IL-17、IL-1β等炎症因子水平,ROC分析miR-223、miR-155对RA疾病的诊断价值,Pearson检验分析miR-223、miR-155与血清炎症因子水平的相关性。结果:RA患者血清外泌体miR-223、miR-155水平显著高于对照者(P<0.05);miR-223、miR-155对RA疾病诊断的AUC为0.842、0.764(95%CI:0.754~0.931、95%CI:0.635~0.857,P<0.05);RA患者血清TNF-α、IL-6、IL-17、IL-1β水平明显高于对照者(P<0.05);血清外泌体miR-223、miR-155水平与RA患者DAS28评分、血清抗CCP抗体、CRP、ESR、RF水平、TNF-α、IL-6、IL-17、IL-1β正相关。结论:miR-223、miR-155在RA患者血清外泌体中表达上调,与RA疾病发生和严重程度有关。Objective:To detect the expression of miR-223 and miR-155 in serum exosomes of rheumatoid arthritis(RA) patients and its clinical significance.Methods: 77 RA patients addmitted to our hospital from April 2015 to August 2017 were studied,and 75 healthy people who underwent physical examination in our hospital during the same period were selected as controls.The levels of miR-223 and miR-155 in serum exosomes were detected by real time fluorescence quantitative(qRT)-PCR.The levels of serum tumor immune factor(TNF)-α,IL-6,IL-17,IL-1β and other inflammatory factors were detected by ELISA kit.The diagnostic value of miR-223 and miR-155 for RA disease was analyzed by ROC.The correlation between miR-223,miR-155 with serum inflammatory factor levels was analyzed by Pearson test.Results: The levels of miR-223 and miR-155 in serum exosomes of RA patients were much higher than heathy controls(P<0.05).The AUC of serum exosomes miR-223 and miR-155 for the diagnosis of RA was 0.842(95%CI:0.754-0.931) and 0.764(95%CI:0.635-0.857).The levels of serum TNF-α,IL-6,IL-17 and IL-1β in RA patients were significantly higher than healthy controls(P<0.05).The levels of miR-223 and miR-155 in serum exosomes were positively correlated with DAS28 score,serum anti-CCP antibody,CRP,ESR,RF levels,TNF-α,IL-6,IL-17 and IL-1β in RA patients.Conclusion: The expression of miR-223 and miR-155 is up-regulated in serum exosomes of RA patients,which is related to the occurrence and severity of RA disease.
