摘 要:目的探究C臂定位下经皮椎弓根螺钉植入术治疗胸腰椎骨折的临床效果。方法选取52例胸腰椎骨折患者于2017年1月至2018年6月展开治疗,依据接诊顺序的奇偶性将其划分为观察组和对照组,各26例。观察组选择C臂定位下经皮椎弓根螺钉植入术治疗,对照组选择后路椎弓根螺钉内固定术治疗。比较两组的椎体影像学指标和围手术期指标。结果术后1年,两组患者的后凸Cobb角均小于术前,椎体前缘高度比值均大于术前(P<0.05);术后1年,两组的后凸Cobb角和椎体前缘高度比值比较,差异无统计学意义(P>0.05)。两组均无神经损伤、椎弓根钉误入椎管等严重并发症的出现,顺利完成手术,术后未发生断裂、内固定松动等情况。观察组术后离床时间、手术时间、住院时间及出血量均优于对照组(P<0.05)。结论针对胸腰椎骨折患者采用C臂定位下经皮椎弓根螺钉植入术治疗,可取得较佳的治疗效果,能加快患者的康复速度,显著改善其生活质量,临床应用价值高。Objective To investigate the clinical effect of percutaneous pedicle screw implantation under C-arm positioning in the treatment of thoracolumbar fracture. Methods A total of 52 patients with thoracolumbar fracture were selected for treatment from January 2017 to June 2018. According to the parity of the reception sequence, the patients were divided into observation group and control group, with 26 cases in each group. The observation group was treated with percutaneous pedicle screw implantation under C-arm positioning, and the control group was treated with posterior pedicle screw internal fixation. The vertebral body imaging indexes and perioperative indexes of the two groups were compared.Results At one year after operation, the kyphosis Cobb angle of the two groups were smaller than those before operation,and the height ratio of the anterior edge of the vertebral body was greater than that before operation(P<0.05). At one year after operation, there were no significant differences in the kyphosis Cobb angle and the height ratio of the anterior edge of the vertebral body between the two groups(P >0.05). There were no serious complications such as nerve injury, pedicle screw into spinal canal by mistake in both groups. The operation was successfully completed, and no fracture or loosening of internal fixation occurred after operation. The time of leaving the bed, operation time, hospitalization time and the amount of bleeding volume in the observation group were better than those in the control group(P<0.05). Conclusion For the patients with thoracolumbar fracture, percutaneous pedicle screw implantation under the C-arm positioning can achieve better therapeutic effect, accelerate the recovery speed of patients, and significantly improve their quality of life, with high clinical application value.
